FDEP Facility ID No. 109500594

Dear Mr. Talley, Please find attached Geosyntec’s Task Assignment Change Order (TACO) #3 for the Year 2 Underground Injection Control (UIC) and annual groundwater monitoring activities, outlined below, for the Former Bellair Cleaners (Facility ID 109500594) located in Orange Park, Florida.

A summary of the scope of work and costs associated with TACO #1 is provided below in red text.

Subtask 1 – Project Management

Subtask 2 – Year 2, Quarter 2 UIC Groundwater Monitoring: UIC groundwater samples will be collected semi-annually from upgradient monitoring well MW002, source area monitoring well MW001, and downgradient monitoring well MW004. The UIC groundwater samples will be submitted to a fixed-base laboratory and analyzed for sodium, total dissolved solids (TDS), and total recoverable petroleum hydrocarbons (TRPH). Chlorinated volatile organic compounds (CVOC), total organic carbon (TOC), ethane, and ethene will also be analyzed for during the semi-annual events to monitor reductive dechlorination following placement of the amendment. The samples will be stored on ice and shipped to the analytical laboratory under chain of custody for analysis under standard turnaround times. Geosyntec will containerize groundwater IDW in one 55-gallon steel drum. Based on historical monitoring well contaminant concentrations, the IDW drum will be sampled for CVOCs to confirm compliance with the existing non-hazardous waste profile with PermaFix. IDW T&D costs have been included in Subtask 4. Groundwater sampling logs will be submitted to FDEP to document the field work for invoicing. ADaPT and GIS datatables from Year 2, Quarter 2 will be compiled after one year of monitoring and submitted with the annual report, discussed in Subtask 4.

Mr. Killian Talley 29 July 2019

Subtask 3 – Year 2, Quarter 4 UIC and Annual Groundwater Monitoring: UIC groundwater samples will be collected semi-annually from upgradient monitoring well MW002, source area monitoring well MW001, and downgradient monitoring well MW004. The UIC groundwater samples will be submitted to a fixed-base laboratory and analyzed for sodium, TDS, and TRPH.

TOC will also be analyzed during the semi-annual event to monitor amendment distribution following injection activities. In addition, Geosyntec personnel will collect samples from MW001, MW002, MW003, MW004, MW005, MW006, MW007, and MW008 for CVOCs, ethane, and ethene during the annual groundwater monitoring event to evaluate the dechlorination progression within the surficial aquifer. Prior to groundwater sampling, Geosyntec will gauge and record the depth to water from site monitoring wells to document groundwater flow conditions. The samples will be stored on ice and shipped to the analytical laboratory under chain of custody for analysis under standard turnaround times. Geosyntec will containerize groundwater IDW in a 55-gallon steel drum. The IDW drum will be sampled for CVOCs to confirm compliance with the existing non-hazardous waste profile with PermaFix. IDW T&D costs have been included in Subtask 4.

WCD #2: As was discussed with the FDEP during the path forward teleconference in March 2019, Geosytnec collected an additional groundwater sample from just above the hardpan in monitoring well MW004 to be analyzed for CVOCs. The additional costs associated with the additional groundwater sample collection are reflected in TACO 2.

The costs associated with WCD #2 are below the authorized amount.

Subtask 4 – Reporting: Year 2 of UIC monitoring and the annual groundwater sampling event will be documented in an Annual Groundwater Monitoring Report which will be signed and sealed by a licensed Florida engineer or geologist in charge of the project. Copies of final manifests will be included in the report. In addition, the ADaPT files and associated GIS datatables for Year 2 of UIC monitoring and the annual monitoring event will also be submitted.

The FDEP is being reimbursed for the costs associated with the disposal of the IDW purge water drum from the semi-annual groundwater sampling event conducted in September 2018 that was stolen.

The Annual Groundwater Monitoring Report will also document the additional monitoring well installation and groundwater sampling activities discussed in Subtasks 6 and 7. The ADaPT files and associated GIS datatables for the additional groundwater sampling activities associated with the newly installed monitoring wells will also be submitted.

Subtask 5 – Work Change Directive #1: Following the collection of the semi-annual groundwater samples in September 2018, Geosyntec subcontracted PermaFix to manifest and dispose of the IDW purge water drum. When PermaFix arrived at the site on 17 October 2018, they informed Geosyntec that the IDW purge water drum was no longer in the staging area and was unable to be located. Geosyntec contacted the Clay County Sheriff’s office on 19 October 2018 to report the missing drum and were informed that a statement would need to be given at the site and a police report would need to be prepared. The additional costs associated with the site visit to file the police report for the stolen IDW drum on 19 October 2018 are reflected in TACO 1.

The costs associated with WCD#1 are below the authorized amount.

Subtask 6 – Monitoring Well Installations: Subtask 6 includes costs associated with the installation of five monitoring wells located in the driveway and wooded area south of the Site.

Prior to conducting field activities associated with the well installation, a site boundary survey and wetland survey of the wooded area behind will be conducted. The proposed monitoring well locations will be cleared and surveyed for underground utilities by the appropriate subcontractors prior to drilling activities.

As was discussed with the FDEP during the January 2019 meeting and subsequent path forward teleconference in March 2019, the proposed monitoring wells will be installed south of the Site building in the alleyway and in the wooded area along the southern property boundary at the locations presented on Figure 1. Three shallow monitoring wells (MW009, MW010, and MW011) will be installed in the alley and two monitoring wells (one shallow [MW012] and one deep [MW013]) will be installed south of MW006 along the property boundary. Each well will be installed using the direct push technology (DPT) drilling method and constructed of 1-inch (in) diameter Schedule-40 polyvinyl chloride (PVC) pipe with 5-feet (ft) of 0.010-in. pre-packed standard factory-slotted screen. Monitoring wells MW009, MW010, MW011, and MW012 will be screened above the hard pan layer noted in the rear of the former dry cleaning facility (approximately 16-ft below land surface (BLS)). The proposed vertical extent well (MW013) will be installed using the direct push DPT drilling method and constructed of 1-in diameter Schedule- 40 PVC pipe with 5-ft of 0.010-in. pre-packed standard factory-slotted screen in the same geologic unit as MW003 (screened beneath a second hard pan unit observed at approximately 40-ft BLS to a total depth of approximately 51-ft BLS). The final monitoring well depths may be adjusted based on observed lithology and confirmed with FDEP. Monitoring wells MW009, MW010, and MW011 will be completed at land surface with flush-mount manholes, 2-ft by 2-ft by 4-in. thick concrete pads, and locking monitoring well caps. Monitoring wells MW012 and MW013 will be completed at land surface with 4-ft aluminum stick-up protective casings set in 2-ft by 2-ft by 4-in thick concrete pads. The monitoring wells will be developed until the discharge is relatively sediment-free. Monitoring well locations and top of casing elevations will be surveyed by a licensed Florida surveyor.

Bid costs prepared by the land clearing subcontractors, private utility locaters, DPT drillers, and surveyor are attached and summarized in the following table. Based on the bids, Geosyntec recommends selecting ERS for the land clearing, GeoTek to complete the utility locate and Preferred Drilling Solutions (PDS) to complete the monitoring well installation activities. Multiple bids were not obtained for the survey activities as it is considered a professional service.

Geosyntec has selected Southeastern Surveying and Environmental Resource Solutions to complete the survey activities.

Land Clearing Moran $3,335

FECC $4,073

ERS $2,785

Utility Locate GeoTek Services $750

Soil, purge water, and decontamination water generated during monitoring well installation activities will be placed in labeled, 55-gallon steel drums and manifested for off-Site disposal under the existing non-hazardous waste profile. It is estimated that five non-hazardous drums will be generated. IDW will be staged along the edge of the alleyway in the rear of the former dry cleaning facility and transported off-Site by PermaFix Environmental Services.

Upon completion of the monitoring well installation activities, Geosyntec will submit a field package consisting of field notes, boring logs, and monitoring well construction logs.

The items for the costs reduction associated with the reduction in IDW generated during the monitoring well installation activities. Due to scheduling conflicts, the monitoring well installation activities were completed by different Geosyntec personnel. The costs associated with the staffing adjustment is reflected in TACO 3.

Subtask 7 – New Monitoring Well Groundwater Sampling: Following the installation of the five new site monitoring wells, Geosyntec personnel will collect samples from MW009, MW010, MW011, MW012, and MW013 for CVOCs, ethane, and ethene to evaluate the dechlorination progression within the surficial aquifer and refine the limits of the current groundwater contaminant plume. Prior to groundwater sampling, Geosyntec will gauge and record the depth to water from site monitoring wells to document groundwater flow conditions. The samples will be stored on ice and shipped to the analytical laboratory under chain of custody for analysis under standard turnaround times.

Due to scheduling conflicts, the groundwater sampling activities were completed by different Geosyntec personnel. Additionally, as the groundwater sampling event was conducted in one day, the FDEP is having costs reduced for the second day’s mobilization time. The costs associated with the staffing adjustment and reduction in mobilization time are reflected in TACO 3.
