The assignment type is designated as a Fixed Price contract with no retainage.
The task assignment authorizes Geosyntec to conduct per- and polyfluoroalkyl substance (PFAS) groundwater sampling activities, consisting of three subtasks: Project Management, Groundwater Sampling, and IDW Management. Groundwater samples will be collected from three monitoring wells (MW-01, IMW-01, and IMW-03) in October 2021 and analyzed by the FDEP laboratory for PFAS constituents. The deliverables include groundwater sampling field documentation due November 10, 2021 ($2,655.88) and a final IDW manifest due December 31, 2021 ($535.42). The total task assignment value is $3,191.30, with a final task completion date of March 31, 2022. IDW (investigative derived waste) transport and disposal for one 55-gallon drum will be managed by Hulls Environmental Services at a cost of $175.00 per drum.

This Task Assignment (TA) authorizes Geosyntec to perform per- and polyfluoroalkyl substance (PFAS) groundwater sampling activities and generate a field report.
Subtask 1: An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtask 2: Under this subtask Geosyntec will mobilize to site for polyfluoroalkyl substance (PFAS) groundwater monitoring activities from three (MW-01, IMW-01, and IMW-03 ). Samples will be sent to the FDEP laboratory who will analyze the three groundwater samples for PFAS constituents. Two equipment rinsates will be collected for peristaltic and submersible pump samples. FDEP will provide PFAS-free water for the equipment rinsates and will deliver the PFAS-free water with the sampling kits.
Geosyntec will submit an emailed summary as a deliverable, which will include a PFOS, PFOA, and PFOS+PFOA concentration figure, field notes/forms, and the certified lab report.
Subtask 3: This subtask includes costs for IDW transport and disposal for one 55-gallon drum that will be disposed off site by Hulls Environmental Services.
The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment (TA), total task amount is $3,191.30
The final task completion date is March 31, 2022

Dear Ms. Posey, Please find attached Geosyntec’s Scope of Work (SOW) and Cost Estimate Form and proposed Level of Effort (CEF/LOE) for per- and polyfluoroalkyl substance (PFAS) groundwater sampling activities at Art Craft Cleaners (Site) located in Jacksonville, Florida. A summary of the costs associated with the attached CEF/LOE is provided below.
Subtask 1 – Project Management
Subtask 2 – Groundwater Sampling
Subtask 2 includes costs associated with PFAS groundwater screening at the Site which is planned for October 2021. As requested by FDEP via email on 31 August and 2 September 2021, groundwater samples will be collected from source area monitoring well MW-01 and downgradient monitoring wells IMW-01 and IMW-03. Groundwater sampling activities will be conducted in general accordance with Standard Operation Procedures (SOPs) for PFAS sampling that were developed by Geosyntec. The sample from MW-01 will be collected using conventional low flow purging techniques with a peristaltic pump. At IMW-01 and IMW-03, the existing passive diffusion bag (PDB) samplers will be removed from the well, placed on 6-mil plastic sheeting, and a well volume will be purged using a submersible pump (Grundfos Redi-Flo 2.0 as previously approved by FDEP for PFAS sampling) prior to sampling to mitigate potential cross-contamination from the PDBs. Following sampling, the PDBs will be re-deployed for the 2022 annual groundwater sampling event.
Art Craft_DC026J_2021 PFAS GW Sampling_SOW
Geosyntec understands that the FDEP laboratory will analyze the three groundwater samples for PFAS constituents, and the FDEP will ship the sampling kits to Geosyntec. Based on the location of the Site and duration of the work, Geosyntec anticipates shipping the samples to the FDEP laboratory, unless other arrangements are made with FDEP.
Based on FDEP’s email request dated, 10 September 2021, Geosyntec will collect two equipment rinsates (1 rinsate each for the peristaltic and submersible pump samples). Geosyntec understands that the FDEP will provide PFAS-free water for the equipment rinsates and will deliver the PFAS-free water with the sampling kits.
Geosyntec will submit an emailed summary as a deliverable, which will include a monitoring well analytical results figure illustrating PFOS, PFOA, and PFOS+PFOA concentrations in text boxes (without isoconcentration contours), field notes/forms, and the certified lab report (i.e., “final”) from the DEP laboratory.
Subtask 3 – IDW Management Subtask 3 includes costs for IDW transport and disposal. Purge water and decontamination water generated during sampling activities will be placed in one new 55-gallon drum and manifested for off-site disposal by Hulls Environmental Services. The monitoring well sampling data will be used to confirm compliance with the existing non-hazardous waste profile. The FDEP requested that the IDW be sent to a landfill for disposal at other PFAS assessment sites. The Hulls quote is included as Attachment A.
Due to Hulls proximity to the Site, we have assumed that no Geosyntec oversight is required for drum transportation and disposal.
Invoice and Deliverable Schedule:
Subtask Deliverable Deliverable Due Date Invoice Amount
Submittal of groundwater sampling field documentation 10 November 2021 $2,655.88
1 & 3 Submittal of final IDW manifest 31 December 2021 $535.42
Total $3,191.30
As always, it is a pleasure to work with the FDEP. Please contact us if you have any questions or require additional information.
