About this file

This document is a Task Assignment Notification Form for contract HW550 issued by the Florida Department of Environmental Protection (FDEP) to Geosyntec Consultants for conducting annual groundwater monitoring activities at the Lipham Cleaners site located at 2492 West State Road 434 in Longwood, Seminole County, Florida (FDEP Facility ID ERIC_5500). The task assignment, numbered DC010J, was executed on December 30, 2019, with FDEP Project Manager Killian Talley overseeing the engagement and Mike Lodato serving as the Contractor Representative for Geosyntec Consultants.

The task assignment authorizes Geosyntec Consultants to conduct annual groundwater sampling activities and prepare a comprehensive monitoring report for a fixed price of $8,225.33. The scope of work comprises three subtasks: Project Management (Subtask 1), Annual Groundwater Monitoring (Subtask 2), and Groundwater Monitoring Report preparation (Subtask 3). Subtask 2 includes mobilization to collect depth-to-water measurements from 30 monitoring wells and groundwater samples from five designated monitoring wells (MW009, MW021, MW025, MW026, and MW029), with samples analyzed via EPA Method 8260B for volatile organic compounds. Investigative derived waste disposal is subcontracted to Universal Environmental Solutions at $150 per drum. The deliverable schedule specifies submission of field work completion documentation by February 28, 2020, and the final Groundwater Monitoring Report with ADaPT/GIS updates and IDW Manifest by March 30, 2020, with performance completion through July 17, 2020. The total task value of $8,225.33 is invoiced in two installments: $4,246.39 upon receipt of field work documentation and $3,978.94 upon FDEP receipt and approval of the final monitoring report.

From: Killian Talley, Project Manager Date: December 20, 2019 Subject: Negotiation Summary

HW550 DC010J

Task Assignment HW550 DC010J is for Geosyntec to perform annual groundwater sampling.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2: Annual Groundwater Monitoring Subtask includes costs and labor associated with the completion of annual groundwater monitoring event. Geosyntec will mobilize to site for collecting depth to water measurements, five groundwater samples from monitoring wells (MW009, MW021, MW025, MW026, and MW029), and geochemical data. Samples will be sent to a fixed base laboratory and sampled via EPA Method 8260B. Investigative derived waste will be transported and disposed by Universal Environmental Solutions who has been selected as the lowest responsive and responsible bidder.

Subtask 3: Groundwater Monitoring Report Upon receipt of the groundwater analytical data, Geosyntec will generate a Groundwater Monitoring Report outlining and describing the field event, procedures, and analytical results.

The report will also provide conclusions and recommendations. One electronic copy of the final report will be submitted to FDEP (including ADaPT and updated FDEP GIS tables) by email and uploaded to the FDEP FTP website.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $8,225.33 with 69 labor hours.

Geosyntec Consultants may invoice following the schedule established in the Deliverables and Invoice Schedule Attachment, which accompanies the Task Assignment Notification Form.

Lipham Cleaners_Annual GW Sampling_SOW_DC010J_2019-12

10 December 2019

Mr. Killian Talley Florida Department of Environmental Protection Waste Cleanup Program 2600 Blair Stone Road, MS 4520 Tallahassee, FL 32399-2400

Subject: Proposal for 2020 Annual Groundwater Monitoring Lipham Cleaners 2492 W State Road 434, Longwood, Seminole County, Florida FDEP Facility ID ERIC_5500

Dear Mr. Talley:

Geosyntec has prepared the attached Scope of Work and proposed Level of Effort (LOE) for annual groundwater monitoring at Lipham Cleaners (Site) located in Longwood, Orange County, Florida. A detailed explanation of the proposed scope of work and report preparation is provided below.

Subtask 1 – Project Management

This subtask covers the general management of the project as specified in Contract No. HW550.

Subtask 2 – Annual Groundwater Monitoring:

This subtask includes the labor and expenses for the completion of an annual groundwater monitoring event. Geosyntec will mobilize to the Site in January 2020 to collect depth to groundwater measurements from Site-wide monitoring wells (a total of 30 monitoring wells).

Geosyntec will also collect groundwater samples from five monitoring wells (MW009, MW021, MW025, MW026, and MW029). As part of the 2019 Annual Groundwater Monitoring Report, Geosyntec recommended (and FDEP agreed) removal of monitoring wells MW028 and MW031 from the monitoring well network for future sampling events (results below Groundwater Cleanup Target Levels for at least two consecutive sampling events). One duplicate groundwater sample will be collected for quality assurance purposes. Groundwater samples will be submitted for laboratory analysis of volatile organic halocarbons (VOHs) by Environmental Protection Agency (EPA) Method 8260B. Geosyntec will collect field parameters (consisting of pH, specific conductance, temperature, dissolved oxygen, and oxidation-reduction potential) using a YSI water quality meter during groundwater sampling activities.

Mr. Killian Talley 10 December 2019

Lipham Cleaners_Annual GW Sampling_SOW_DC010J_2019-12

Groundwater sampling purge water will be containerized in 55-gallon drums during sampling activities. For purposes of this proposal, the cost for transportation and disposal of one 55-gallon drum of investigative derived waste (IDW) as non-hazardous waste is included in the LOE. This cost is included under Subtask 3 for deliverable and invoicing purposes. Universal Environmental Solutions, LLC is the selected subcontractor for IDW transport and disposal. Subcontractor bids are included in Attachment A.

Subtask 3 – Groundwater Monitoring Report:

Geosyntec will prepare a report documenting the results of the depth to groundwater measurements and annual groundwater monitoring event. The report will contain tables summarizing field/laboratory groundwater data, figures showing groundwater flow direction(s), and a summary of the groundwater monitoring event. The report will be signed and sealed by a Florida-licensed professional engineer or geologist. Effort included in this subtask includes the expected effort needed to provide FDEP with updated ADaPT and GIS data tables consisting of groundwater elevation measurements and groundwater sampling laboratory results.

Invoice and Deliverable Schedule:

Subtask Deliverable Deliverable Due Date Invoice Amount

Submittal of notification of completion of groundwater sampling field work

28 February 2020 $4,246.39
