This is a Task Assignment Notification Form under Florida Department of Environmental Protection (FDEP) Contract No. HW550, issued on February 12, 2021. The contract authorizes Geosyntec Consultants, Inc. to conduct annual groundwater monitoring activities at the Former Penthouse Cleaners site located at 2442 South Volusia Avenue, Orange City, Volusia County, Florida (FDEP Facility ID ERIC_5539). Crystal Posey serves as the FDEP Contract Manager, with Todd Kafka representing Geosyntec as the contractor representative. The task is classified as a monitoring phase activity under the indefinite delivery vehicle arrangement.
The task assignment is structured as a fixed price contract with a total value of $6,745.25 and zero retainage. The scope of work, which incorporates the October 16, 2020 cost estimate, encompasses three subtasks: project management, groundwater sampling from seven monitoring wells using passive diffusion bags (PDBs) with volatile organic halocarbons analysis, and preparation of an annual monitoring report with ADaPT and GIS data tables. The deliverable—submittal of the Annual Monitoring Report, ADaPT and GIS data tables—is due June 30, 2021, with final task completion by November 30, 2021. Invoice payment of the full $6,745.25 amount is conditional upon FDEP receipt and approval of all deliverables. The cost estimate details labor costs ($4,699.63), overhead ($2,976.26), travel expenses ($194.00), laboratory analysis ($632.00), field expenses ($663.50), and a completion fee ($469.96).

Subject: Scope of Work and Level of Effort Annual Groundwater Monitoring Former Penthouse Cleaners 2442 South Volusia Avenue Orange City, Volusia County, Florida FDEP Facility ID ERIC_5539
Dear Mr. Talley:
Geosyntec Consultants, Inc. (Geosyntec) has prepared this scope of work (SOW) and level of effort (LOE) for annual groundwater monitoring at the Former Penthouse Cleaners (the “Site”;
Facility ID ERIC_5539) located at 2442 South Volusia Avenue in Volusia County, Orange City, FL. This SOW/LOE incorporates comments provided by the Florida Department of Environmental Protection (FDEP) in review of the Annual Groundwater Monitoring Report - 2019 dated 13 December 2019 (Report). The SOW consists of three subtasks and is summarized below.
Subtask 1 – Project Management
Subtask 2 – Groundwater Sampling
Geosyntec will mobilize to the Site in November 2020. As recommended in the 2019 Report, Geosyntec will collect groundwater samples for volatile organic halocarbons (VOHs) analyses from five monitoring wells using passive diffusion bags (PDBs); as a result, the 2020 sampling event will require two mobilizations to the Site – one to deploy the PDBs and the second to conduct sampling. Thereafter, the PDBs will be deployed annually at the conclusion of the sampling event during a single mobilization. The monitoring wells include three Upper Floridan aquifer wells (MW001, MW004, and MW008) and two lower surficial wells (MW005 and MW006). Additionally, per email communication with the FDEP in March 2020, Geosyntec recommends sampling MW013 (lower surficial) and MW014 (upper surficial) to evaluate the lateral extent of VOH concentrations at MW005. During the first mobilization, Geosyntec will deploy new PDBs in MW001, MW004, MW005, MW006, MW008, MW013, and MW014 at least two weeks prior to sampling to allow the PDBs to equilibrate with the aquifer. During the second mobilization, Geosyntec will collect a synoptic round of groundwater levels from 15 existing monitoring wells at the Site, collect groundwater samples from the seven recently deployed PDBs, and redeploy new PDBs at MW001, MW004, MW005, the groundwater sampling has been delayed and will be completed in 2021 - CP http://www.geosyntec.com/ Posey_C Cross-Out
Mr. Killian Talley 16 October 2020
FR0857F/PENTHOUSE CLEANERS NAM LOE 2020_FINAL
MW006, MW008, MW013, and MW014 for annual groundwater sampling in 2021. Water quality parameters will not be collected as part of PBD sampling. Groundwater samples will be submitted for laboratory analysis of VOHs by Environmental Protection Agency Method 8260B. One field duplicate sample will be collected for quality assurance/quality control purposes. IDW will not be collected or containerized during sampling activities. Water remaining in the PDBs after sampling will be returned to the monitoring well from which is was pulled.
Geosyntec has assumed that this subtask can be completed in two field days including round trip travel, with at least two weeks between the first and second mobilizations.
Subtask 3 – Annual Groundwater Monitoring Report Preparation
Geosyntec will prepare a report summarizing sampling activities and results, and provide conclusions and recommendations based on data evaluation. The report will contain field forms, tables summarizing field and laboratory data, figures showing groundwater flow directions, and a textual summary of the groundwater monitoring event. The PCE groundwater isocontour map will be updated with new analytical results. The report will be signed and sealed by a Florida-licensed geologist and one electronic copy of the final report will be submitted to the FDEP via email and uploaded to FDEP’s file transfer protocol website.
This task also includes preparation of ADaPT and GIS data tables.
Invoice and Deliverable Schedule:
Note that this schedule reflects two weeks FDEP review of the LOE and the intervening November and December holidays. Geosyntec requests the following schedule of deliverables and invoices:
