Summary
This is a Task Assignment Change Order Form issued by the Florida Department of Environmental Protection (FDEP) for Contract No. CN213. The document modifies Task Assignment No. 20 between FDEP and HALFF Associates, Inc., a consulting firm. The project involves the Ravine Gardens State Park Stormwater Solution, with David Hutcheson, P.E. serving as the consultant representative and Ralph Perkins as the FDEP Contract Manager. All required approvals were digitally signed between November 16 and November 21, 2023.
The change order extends the task execution period by 365 days, with the new task end date of December 30, 2024, to provide limited construction administration services as detailed in a proposal dated August 14, 2023. The extension was necessitated by project rebidding. The fixed fee for this change order is $15,260.00, with no reimbursable expenses. The original task had no previous change orders, and the total task amount with all change orders remains $15,260.00. The original task execution date was September 1, 2022, with an initial end date of December 31, 2023.
