Clinical Critical Incidents Q3/Q4 FY 2024-2025

Attached are the Critical Clinical Incident (CCI) logs for Q3 and Q4 of FY 2024-2025. According to the contract, 2.4.46 Required Committee Meetings, Quality Assurance Committee: "For any CCI event review that results in a Category Four ( 4) the Vendor shall credit the IDOC $10,000.00. This credit is to reimburse the IDOC for additional action, oversight, and review expended by the IDOC in dealing administratively with such findings. This reimbursement shall be paid as a credit on the next invoice due the IDOC after notification." As the logs indicate, there were eight (8) Category Four events from January I, 2025 through June 30, 2025. They are as follows:
