Blk 4: Tailoring of DID Requirements. The contractor shall report all activities at the WBS level III (work package level). Technical and cost status information shall be included in this report. Contractor format is acceptable, upon Government approval. In addition, the contractor shall prepare labor reports at the Work Unit level and provide a line item report of all commitments, obligations, and expenditures within each project. Must be compatible with Microsoft Office 2010 or later.

Blks 10-13: The contractor shall deliver the first Status Report within 15 calendar days after completion of the first full contractor accounting quarter. The subsequent status reports and the line item reports shall be delivered within 15 calendar days after the end of each subsequent accounting period (quarterly) thereafter through technical completion.

However, following the conclusion of a major analysis/activity a technical report shall be generated. Must be compatible with Microsoft Office 2010 or later version.

Blks 8, 10-13: Technical report for major analysis/activity shall be due 30 days after the major analysis/activity is completed. Draft quarterly report due 15 days after reporting period. The Government will provide comments to the draft within 10 days of receipt. The contractor shall submit the final version within 10 days of receipt of Government comments.

THE REPORT WILL BE PUBLISHED WITH THE DEFENSE TECHNICAL INFORMATION CENTER (DTIC) BY AFRL STINFO OFFICE. Under no circumstances shall the contractor submit any version of the scientific and technical report to DTIC.
Blk 4: GUIDANCE FOR PREPARATION OF THE REPORT: The Scientific and Technical Report shall be prepared I.A.W. the most current AFRL ANSI/NISO Standard Z39.18-2005, or other most current report preparation guidance. The contractor shall complete the most current SF 298 Report Documentation Page, and submit it with the report.
The contractor shall complete the following blocks of the SF 298: Blocks 2-5a, 6, 7, 8 and 13 (if applicable), and 14-16c. The contractor shall complete and sign the most current AFRL Form 25, AFRL Quality Final Technical Report Checklist, and submit it with the report as part of the report deliverable IAW CLIN ____. If the completed and signed AFRL Form 25 does not accompany the report, the deliverable is considered incomplete. Incomplete reports will be immediately returned to the contractor. Following AFRL technical and STINFO reviews, any revisions required will be returned to the contractor. NOTE: The report preparation guidance, SF 298, and AFRL Form 25 are available at:
http://www.kirtland.af.mil/prs.
The contractor shall deliver two editable electronic copies of the report. The electronic copies shall be an editable .pdf file and the most current version of Microsoft Word. A CD- ROM (CD-R) is the acceptable media for submittal (CD-RWs are not acceptable). Electronic media shall be marked I.A.W. the AFRL ANSI/NISO Standard Z39.18-2005 guidelines, or other most current report preparation guidance. Submission via unencrypted or unsecure email is not permitted.
Blk 7: UPON NOTIFICATION OF ACCEPTANCE OF THE SCIENTIFIC AND TECHNICAL REPORT FROM THE PROGRAM OFFICE, the contractor shall submit the final Receiving Report (DD Form 250) in WAWF. In preparing the final DD Form 250, code the shipment number with a “Z.” Do not upload the Scientific and Technical Report into WAWF or DTIC. NOTE: SUBMITTAL OF THE DD FORM 250Z PRIOR TO ACCEPTANCE NOTIFICATION WILL RESULT IN REJECTION OF THE DD FORM
250Z FINAL RECEIVING REPORT.

Blk 4: The CAP Property List shall be prepared on a spreadsheet using Microsoft Office Excel in accordance with the remarks below. Abbreviations are not allowed.
Submission via unencrypted or unsecure email is not permitted.
As specified in the contract, the contractor may purchase material or equipment (CAP) using Government funds for performance of the contract.
The CAP Property List shall include at a minimum:
- List of CAP by nomenclature
- National Stock Number (NSN)
- CAP meeting IUID criteria of DFARS 211-274-2
- Serial number of each equipment
- Quantity of each equipment
- Unit of measure of each equipment
- Original Unit Acquisition Cost
- Date placed in service by the Contractor
- Condition of each equipment in accordance with the equipment condition status listed:
- Service Usable without qualification. New, Used, Repaired, or Reconditioned
- Service Usable with qualification. New, used, repaired or reconditioned equipment which is serviceable and useable for its intended purposes but which is restricted for issue to specific units, activities, or geographical areas by reason of its limited usefulness or short service life expectancy.
- Serviceable Test/Modifications. Serviceable equipment which requires test, alteration, modification, conversion or disassembly.
- Solutions of problems relating to the above
- Request for assistance for repair of CAP, as necessary
Blk 9: Distribution Statement A. Approved for public release: Distribution unlimited.
Blk 11: The contractor shall deliver the required annual CAP Property List in accordance with the Statement of Work and/or contract schedule.
Blks 12 & 13: Ninety (90) days prior to the contract expiration date, the contractor shall deliver a “Draft” CAP Property List in final format suitable for contract close-out purposes containing the use/disposition recommendations for review and approval by the Contract Program Manager and/or the DCMA Property Administrator. The Contract Program Manager will review and if necessary, return the list with comments no later than the date specified in the Schedule.
Blk 14: Thirty (30) days prior to the contract expiration date, The contractor shall distribute the “Final” CAP Property List approved by the Contract Program Manager and/or the DCMA Property Administrator in accordance with Blk 14. When the addressee’s copy is identified by “Ltr”, send only a copy of the transmittal letter to that addressee.
