Please complete and submit the following questionnaire directly to blackce@state.gov, with a copy to hilldk@state.gov by [Month, Day, 2021]. Besides basic administrative or demographic information, the offeror shall not suggest how to answer any of the questions. The Offeror has been advised that the past performance information from its written response and this questionnaire may be discussed with you. There is no page limit. Please write “not applicable” if a question does not apply.

What was the contractor’s overall ability to execute the processes they were tasked with during normal operations?
What was the contractor’s overall ability to execute processes during periods of major program initiatives? What was the contractor’s task approach, including communications strategy and time frames, during these periods? Was it aggressive but achievable? Was it effective but efficient? How?
Did the program experience any major changes in direction during contract execution? If so, did they demonstrate the requisite flexibility and execute successfully?
How well was the contractor able to retain quality resources during the period of performance? Did contractor staff turnover affect your implementation?
To what extent was the contractor able to identify and implement timely and relevant process improvements and innovations?
Was the task executed with high quality? What was the contractor’s approach to quality assurance when managing processes? Did the resulting support meet your organization’s expectations?
Did the contractor communicate professionally and appropriately to federal customers and stakeholders?
What level of contractor coordination was required with other vendors in order to execute day-to-day responsibilities? Did the contractor consistently communicate and coordinate effectively with other vendors in a timely, professional manner?
What were some of the problems encountered during the execution of the task? Was the problem the fault of the offeror? How was each problem resolved?
Were there personnel issues on the task? How well was the contractor able to resolve key personnel issues?
Was the project completed within budget? Within the scope of the contract terms, how readily did the contractor’s approach enable scalable program solutions, if at all?
Did the contractor’s level of expertise and experience during the period of performance meet your expectations?
Would you engage the contractor to perform future work of a similar nature and size?
