About this file

This is a Past Performance Questionnaire (PPQ) template used by the VA Program Contracting Activity Central to evaluate contractor performance on past projects relevant to federal contract opportunities. The form is completed by both the contractor being evaluated and the client/evaluator who directly oversaw the contractor's performance on a previous contract. Contractors must submit completed PPQs as part of their proposal, and these documents count toward the page limitations specified in the solicitation's Instructions to Offerors.

The questionnaire requires contractors to provide detailed information about their past contract performance, including contractor details, contract specifications (title, number, type, location, award date, and acceptance dates), project scope, and pricing data (original implementation price, original contract price, and final contract price with modifications). Evaluators assess contractor performance across seven key categories: quality of work, schedule/timeliness of performance, communication and cooperation, management/personnel/labor effectiveness and continuity, cost/financial management, subcontract management, and an overall summary rating. Each category uses a five-point adjectival rating scale (Outstanding, Above Average, Satisfactory, Unsatisfactory, or Not Applicable), with evaluators providing supporting comments for each rating. The completed questionnaire is submitted to the requesting firm prior to the proposal submission deadline specified in the solicitation and is incorporated into the offeror's proposal for evaluation purposes.
