About this file
This document is a Past Performance Questionnaire (PPQ) template issued by the U.S. Department of the Interior, Bureau of Reclamation, Boulder City, NV, for Solicitation No. 140R3026R0012.
The questionnaire is designed to collect evaluations of contractor past performance from references and previous clients. It requests basic contract information including the firm name and address, contract number, type of contract, project title, award and completion dates, location, initial and final contract amounts, and a project description. The form captures evaluator information including name, organization, contact details, and role in relation to the project. Evaluators must rate performance across 14 specific categories using a five-point scale: Outstanding (O), Above Average (A), Satisfactory (S), Marginal (M), and Unacceptable (U). Performance areas evaluated include client relationship management, subcontractor coordination, corporate management and integrity, work quality, quality control, schedule performance, cost control, labor and safety compliance, and overall customer satisfaction. The questionnaire also includes yes/no questions regarding cure notices, termination actions, contract award likelihood, customer satisfaction, and whether the firm was provided opportunity to respond to negative feedback. An additional remarks section allows for supporting comments, particularly when Marginal or Unacceptable ratings are assigned. The completed questionnaire must be submitted via email to colleen_samuels@ios.doi.gov.
