About this file
This is a Contractor's License Information form for installation services required as an attachment to a state and local contract opportunity in California. The document establishes licensing requirements for contractors and subcontractors performing work under the contract, specifically mandating a California C-7 license for any installation work involving cable, wiring, and electrical modifications. Contractors and their subcontractors must obtain all necessary licenses and permits at their own expense prior to commencing work, and the license must be held either by the company or its qualifying individual. The contractor is responsible for maintaining current licensure throughout the contract term, which is subject to verification by the State at any time, and work cannot be performed at or with a PSAP without valid licensing.
The form requires contractors to complete license information for both the primary contractor and any subcontractors, including class designation, license number, licensee name, and expiration date. At minimum, the responsible managing employee or firm representative must be licensed in addition to all subcontractors performing under the contract. The attachment serves as a compliance document to ensure that all parties executing installation services meet California's contractor licensing standards established by the State License Board and Department of Consumer Affairs.
