Subject: Request for Proposal (RFP) Solicitation No.72052122R00004
Amendment No.0001
Dear Prospective Offerors:
USAID/Haiti has completed its review of the questions submitted in response to the subject Request for
Proposal (RFP).
This Amendment No. 01 is hereby issued to make the following changes:
I. RFP COVER LETTER:
Extend the deadline for receipt of Proposals as follows: “Proposals in response to the subject RFP are due no later than January 20, 2022, 3:00 P.M. Haiti Time.
II. SECTION B – SUPPLIES OR SERVICES AND PRICE/COST:
Modify Subsection B.7 PAYMENT OF FIXED FEE to incorporate the following: “Paragraph (i). Any portion of fee left over from situations arising as described in paragraphs g and h of this section will roll over and be available for pay out in subsequent evaluation period(s) and from other Milestones in the Fee
Distribution Schedule.”
III. SECTION F - DELIVERIES OR PERFORMANCE:
a) Modify Subsection F.7 REPORTS AND DELIVERABLES to include the following Security
Reporting Requirements:
i) Emergency Contact Plan
ii) Security Reporting
b) Modify Subsection F.8 REPORTING REQUIREMENTS to include the following:
● “Emergency Contact Plan: The Contractor is required to submit to the COR a copy of the
Contractor’s Emergency Plan covering the Contractor’s expatriate and local staff. This plan must include phone/SMS trees and plans for other forms of communication.”
● “Security Reporting: As part of the overall security requirements, the Contractor must report any security threats and/or incidents verbally or by telephone, immediately to the
Contracting Officer Representative (COR) or Alternate COR (A/COR) in the COR’s absence. Subsequently, a written report must be promptly submitted to the COR (and
A/COR) as well as to the Partner Security Liaison Office (PLSO); the point of contact will be provided at time of post-award briefing. All subcontractors will be required to report any threats/incidents to the prime Contractor, who will immediately notify the COR (and
A/COR and PLSO). At a minimum, a security incident report must contain the name of the company, name of the individual(s), date, time, a description of what happened, where the incident occurred, and any other relevant details surrounding the incident. If this is an ongoing incident, progress reports must be submitted to keep the COR (and A/COR and
PLSO) apprised of the situation.”
IV. SECTION H – SPECIAL CONTRACT REQUIREMENTS:
a) Modify subsection H.22 ADDITIONAL REQUIREMENTS FOR PERSONNEL
COMPENSATION to delete the following paragraph: “Nor may any individual salary or wage, without approval of the Contracting Officer, exceed the employee's current salary or wage, or the highest rate of annual salary or wage received during any full year of the immediately preceding three (3) years.”
b) Subsection H.32 SECURITY CONSIDERATION has been incorporated.
V. SECTION I – CONTRACT CLAUSES
a) Subsection I.5 FAR 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999) has been incorporated.
VI. SECTION L – INSTRUCTIONS, CONDITIONS AND NOTICES TO OFFERORS:
a) This section has been modified to include a Revised clause on NDAA Section 889. Please refer to
Section L.5 of conformed RFP.
b) Subsection L.8 INSTRUCTIONS FOR THE PREPARATION OF THE TECHNICAL
PROPOSAL has been modified to include the Executive Summary (no more than 2 pages) in the list of sections not subject to the page limitation.
Additionally, the following attachments are added to this Amendment:
1) Asset Indicator PIRS
2) Haiti Reforestation Project FY18 Annual Progress Report
3) Haiti Reforestation Project FY19 Annual Progress Report
4) Haiti Reforestation Project FY20 Annual Progress Report
5) USAID/Haiti PMP Indicators – Performance & Context
6) 1.1.3 Percentage of USAID activity participants with improvement in targeted resilience capacities
All other terms and conditions of the Request for Proposal (RFP) remain unchanged.
