About this file
This is a federal contracting form template for Small Business Program Representations (FAR 52.219-1) required to be completed and submitted with a Request for Quote (RFQ).
The provision establishes definitions and representation requirements for various small business categories eligible for federal contract preferences. The acquisition is classified under NAICS code 311991 with a small business size standard of 700 employees. Offerors must represent their status across multiple small business program categories: whether they qualify as a small business concern, women-owned small business (WOSB), economically disadvantaged women-owned small business (EDWOSB), service-disabled veteran-owned small business (SDVOSB), HUBZone small business, and small disadvantaged business (SDB). The form includes checkbox options for offerors to indicate compliance with specific program requirements, including joint venture structures permitted under 13 CFR regulations. The provision contains a notice section that warns offerors of potential criminal penalties (fines and imprisonment) and administrative remedies (suspension and debarment) under 15 U.S.C. 645(d) for misrepresenting small business status to obtain contracts under preference programs established by the Small Business Act. The yellow-highlighted areas on the form require completion before submission with the RFQ response.
