1.0 INTRODUCTION & EXECUTIVE SUMMARY

Mabbett and Associates, Inc. (Mabbett®), with AECOM as sub-contractors, performed surveys for suspect asbestos containing building materials (ACM) and screenings of suspect lead containing building surfaces utilizing an X-Ray Fluorescence (XRF) analyzer. Surveys were performed of selected buildings at the Loma Linda VA Medical Center (Loma Linda VAMC) located at 11201 Benton Street, Loma Linda, CA, under Contract GS-10F-0120T. Site survey work was performed August 22 to September 3, 2016 by appropriately credentialed personnel as required. Collected suspect ACM samples were submitted to an analytical laboratory certified as an environmental testing laboratory pursuant to the provisions of the Health and Safety Code (certification provided in Volume III) for bulk asbestos analysis by polarized light microscopy (PLM) using positive stop methodology. The survey effort involved the collection or screening of the following samples, resulting in the following conclusions:

• 536 bulk samples for suspect ACM were collected in this building.

• Based on laboratory analysis of suspect ACM, eleven (11) of the collected samples contained asbestos greater than 1%.

• Seven (7) additional materials were previously sampled and identified as ACM or Presumed ACM

(PACM.)

• Analytical results identified ten (10) homogeneous materials as trace (<1%) asbestos.

This building report consists of a summary of findings, floor plans that indicate positive sample locations, detailed analytical findings for the specific surveyed building materials, and relevant photos. Laboratory certificates of analysis and field data sheets for this building report are available in Volume III of the Loma Linda VAMC Survey Report.

2.0 BUILDING DESCRIPTION

According to information provided by the VAMC and observations made by the Mabbett team at the time of the survey, Building 1 is the main hospital building which is a four-story building with interstitial spaces above each of the four floors, a basement and seven penthouses, occupying approximately 800,000 square feet. Approximately 40,000 square feet have been abated, therefore approximately 760,000 square feet were inspected, per VAMC.

3.0 ASBESTOS SURVEY

3.1 Sampling Methodology

The Mabbett team performed a physical U.S. Environmental Protection Agency (EPA) Asbestos Hazard and Emergency Response Act (AHERA) type inspection and sampling of suspect materials as required by EPA Asbestos Hazard Emergency Response Act (AHERA) regulations (i.e. 40 CFR 763), OSHA Asbestos Standards (i.e. 29 CFR

1910.1001 & 29 CFR 1926.1101) and the VHA Asbestos Management Directive (VHA Directive 2010-036).

A visual screening inspection was conducted to identify locations of suspect ACM throughout identified areas of the building by Asbestos Hazard and Emergency Response Act (AHERA) Accredited and a California certified asbestos inspector. Only areas that were accessible during the field work phase were inspected. Every effort was made during the initial field survey work to access areas as necessary to complete the survey. However, areas that remained inaccessible to the survey team are indicated in Table 1 below.

© 2017, Mabbett & Associates, Inc. Building 1 - Page 2 of 25 March 30, 2017

Table 1 – Specific Inaccessible Areas Loma Linda VA Medical Center, Building 1 – Basement through 9th floor

Building Floor Room No. Reason Area Was Inaccessible and Survey Impacts, If Applicable

1 1st 1C-23 No access, locked, potential presence of 12” x 12” Floor Tile and Associated Mastic

General areas that were inaccessible or where the survey was limited to visual observation only are identified below:

• Within walls • Within fire doors

• Enclosed pipe/duct chases • Inside mechanical equipment/ductwork

• Above fixed drywall or plaster ceilings without access panel

Refer to CADD Plans for areas not surveyed, per VAMC request, due to renovations.

The following materials were previously sampled or presumed ACM per the contract and information provided during the survey:

• Various 12” x 12” Floor Tiles and Associated Mastic adhesives

• Transite Fume Hoods

Due to these access restrictions, comprehensive pre-demolition ACM surveys should be performed prior to any proposed renovations or maintenance involving inaccessible areas. Roofing materials were not sampled in order to maintain applicable warranties.

Bulk samples were collected of suspect ACM in accordance with US Environmental Protection Agency (EPA) AHERA and the Mabbett Survey Program Standard Operating Procedure (SOP). Bulk suspect ACM sampling was conducted according to the following sampling plan:

a) Surfacing material:

a. At least three bulk samples were collected from each homogeneous area that is 1,000 ft2 or less.

b. At least five bulk samples were collected from each homogeneous area that is greater than 1,000 ft2 but less than or equal to 5,000 ft2.

c. At least seven bulk samples were collected from each homogeneous area that is greater than 5,000 ft2.

b) Thermal system insulation:

a. At least three bulk samples were collected from each homogeneous area of thermal system insulation.

b. At least one bulk sample were collected from each homogeneous area of patched area of thermal system insulation.

c. Sufficient samples were collected from elbows and fittings to determine if it contains ACM.

d. Bulk samples were not collected from any homogeneous area where the state licensed asbestos inspector determined that the thermal system insulation is fiberglass, foam glass, rubber, or other non-ACM.

© 2017, Mabbett & Associates, Inc. Building 1 - Page 3 of 25 March 30, 2017

c) Miscellaneous material:

a. At least one bulk sample was collected from each homogeneous area that is less than 100 ft2.

b. At least three bulk samples was collected from each homogeneous area that is greater than 100 ft2.

3.2 Analytical Methodology

The collected bulk samples were submitted under chain of custody procedures to AmeriSci of Carson, CA. AmeriSci is accredited by the American Industrial Hygiene Association (AIHA) and participates in the National Voluntary Laboratory Accreditation Program (NVLAP Lab Code 200090-0). Bulk samples were analyzed with polarized light microscopy (PLM) using EPA Method 600/R-93/116. Additionally, samples identified by the laboratory to contain trace amounts of asbestos (less than or equal to 1%) were further analyzed using the EPA Point Count 1000 Method;

this analysis was conducted to sufficiently prove that trace samples were not greater than 1%. Copies of the laboratory accreditations are included in Volume III.

Bulk sample results are summarized in Table 4 – Summary of ACM Sampling Results. The EPA and the Occupational Safety and Health Administration (OSHA defines any material that contains greater than one percent (>1%) asbestos as being an ACM. As such, the analytical laboratory identified bulk samples as positive for asbestos that met the regulatory criteria of greater than or equal to 1% asbestos. The results for positive samples are located in Table 2 – Summary of Positive ACM.

3.3 Summary of Asbestos Containing Materials (ACM) Findings

3.3.1 Data Tables, Laboratory Results, and Field Notes

Collected bulk samples confirmed by the analytical laboratory to contain any asbestos and PACM are listed in Table 2

- Summary of Positive ACM Samples. Volume III includes copies of the laboratory certificates of analysis and Inspector Field Data Sheets.

* The hazard assessment scale 1 – 4 utilized is a relative indicator of the risk and need for response/remediation. (1) represents the highest priority (e.g. removal or encapsulation) where as a (4) represents the lowest priority (monitor as part of 6 month O&M program). The rating, assigned by an Asbestos Management Planner, takes into account:

condition, friable vs. non-friable, accessibility, occupancy (e.g. continuous, intermittent or occasional and patients/staff/visitors), potential for air erosion, potential for vibration damage, potential for disturbance / damage (e.g. exposed and in an accessible location), and potential for water damage.

** The value presented is the total square footage of all 12”x12” tiles previously identified as ACM or determined to be ACM in the 2016 Survey. See Table 6 for a breakdown of flooring materials by room.

© 2017, Mabbett & Associates, Inc. Building 1 - Page 8 of 25 March 30, 2017

*** The value presented is the total square footage of all flooring materials with floor tile black mastic or residual floor tile black mastic present beneath either ACM floor tile, non-ACM floor tile or carpet previously identified as ACM or determined to be ACM in the 2016 Survey. See Table 6 for a breakdown of flooring materials by room.

3.3.2 Photographs

Representative photographs of ACM Homogeneous Areas are provided in Appendix B of this report.

3.3.3 CADD Drawings

The location of each collected bulk sample and its abridged sample ID is shown on the CADD drawings in the Figures Section of this report. The sample ID on the drawings has been abridged (by excluding the individual room number from the full sample ID) for aesthetic purposes. Each positive ACM sample location is colored red and marked with an asterisk (*), while positive stops are marked with two asterisks (**). Collected bulk samples that were not positive are identified solely by the abridged sample ID. Building areas containing ACM have been indicated with hatching to identify the location of the identified ACM.

4.0 LEAD SCREENING SURVEY

4.1 Screening Survey Methodology

Mabbett completed a limited lead screening survey of building components including painted building surfaces and ceramic tile. The lead screening survey was performed by trained lead inspectors. The screening survey measured lead concentrations in accessible building surfaces by using a Niton XLp 702AW XRF (serial number 16230) resourced on 15 August 2016. The XRF instrument was calibrated at the frequency specified in the SOP;

the practical detection limit is 0.02 mg/cm2.

4.2 Summary of Lead Screening Survey Findings

4.2.1 Data Tables

A concentration of lead greater than or equal to 1.0 mg/cm2 exceeds HUD residential standards and is an indicator of risk. OSHA does not specify a safe concentration of lead. Painted interior building components screened using an XRF found to contain lead at concentrations greater than 1.0 mg/cm2 have been included in Table 3 below:

Note:

* = Buried “shielding” is the likely source of the detection at these locations. Lead is therefore buried deep inside the wall and these locations should not be treated as, or assumed to contain lead based paint.

In addition, XRF measurements collected during the screening survey of interior building components are included in Table 5 – Summary of All XRF Measurements. Table 5 includes a description of each screened surface and resulting XRF-measured lead concentration.

4.2.2 Photographs

Representative photographs of components identified with > 1.0 mg/cm2 lead content and where in other than intact condition (e.g. fair, peeling, cracking) are provided in Appendix D of this report.

4.2.3 CADD Drawings

Based on the results of the lead screening survey, a table of components identified with > 1.0 mg/cm2 lead content was developed. This table is presented on the CADD drawings for use by the VAMC for exposure assessments and preliminary renovation planning. Based on the results of the survey, these components identified in the CADD table and on the plan should be assumed to be lead containing unless otherwise determined.

4.2.4 Patterns of Lead Based Paint Occurrences

While testing every occurrence of every component on every floor was not possible within the time allowed, the following patterns related to the occurrence of Lead Based Paint (LBP) or other lead containing materials in major components were noted:

• Approximately 90% of the ceramic wall tile colored yellow, brown or tan was found to contain lead at a concentration at or above 1mg/cm2; this compares with approximately 12% of the ceramic tile colored white.

• Other colors of ceramic tiles were found to contain lead above 1.0 mg/cm2 for approximately 25% of the surfaces tested.

• Excluding walls composed of drywall that were known to be under laid by radiation shielding, less than 5% of the drywall surfaces were found to be coated with LBP.

• Less than 10% of stucco surfaces found on the exterior of the main building and the penthouse tested showed the presence of lead at concentrations at or above 1.0 mg/cm2; this is not thought to be associated with the paint or colorant used to give the stucco its tan hue, but rather, is likely associated with something in…
