About this file

This is a Past Performance Questionnaire for Solicitation No. 140P2026R0078 (YELL 326141 Yellowstone Modular Housing) issued by the National Park Service Denver Service Center. The questionnaire is used to evaluate contractor performance on past projects and must be completed in two phases: offerors complete Sections A and B with their company information and details of a relevant past project, then transmit to a reference who completes Sections C and D providing performance ratings.

The questionnaire evaluates past performance across six key areas: Quality (workmanship, quality control adherence, and technical submittals); Schedule and Time Management (milestone achievement, responsiveness, and timely submissions); Cost Control (handling of site conditions, modification pricing, invoice accuracy, and supplier payments); Management (project oversight, subcontractor coordination, safety, location factor management, and problem-solving); Small Business Subcontracting and Labor Standards (regulatory compliance, socioeconomically disadvantaged business engagement, and subcontracting plan adherence); and Regulatory Compliance (building and life safety codes). References rate performance in each area using a five-point scale from Exceptional to Unsatisfactory and provide supporting comments. The completed questionnaire must be returned to Contracting Officer Vicki Freese-Supler or Contract Specialist Jeanna Manseau at the National Park Service Denver Service Center by September 17, 2026, at 2:00 p.m. MT. This questionnaire serves as source selection sensitive information and cannot be used for promotional purposes by contractors.
