About this file
This is a Past Performance Questionnaire (PPQ) template for the FAA Office of Information Technology's solicitation 697DCK-25-R-00368 for printer consumables under the SAVES (Strategic Sourcing for the Acquisition of Various Supplies and Equipment) program. The form is designed to be completed by references (prior government or commercial clients) who have worked with an offeror being considered for a five-year IDIQ contract. References must submit completed questionnaires directly to Kay F. Morello and Stephen Young at the FAA by 5:00 PM ET on September 21, 2026. Alternatively, official government performance evaluations may be submitted in lieu of the form.
The PPQ requests references to evaluate offeror performance across multiple criteria including relationship with the client team, quality of work and quality control, ability to meet performance schedules, cost control, and timely documentation submission. The questionnaire includes specific evaluation questions regarding the offeror's experience delivering over 750 printer consumables to the 48 contiguous states and non-foreign outside continental U.S. locations, with a required four-business-day delivery timeline for CONUS and OCONUS orders and a performance threshold of 98% on-time delivery. References must assign ratings using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, or Unacceptable) without modification, and provide an overall firm rating and recommendation regarding future contract awards. The government reserves the right to verify all information provided.
