About this file
This is a Past Experience Information Form template designed for offerors responding to the FAA's printer consumables solicitation (697DCK-25-R-00368). The form is used to document and demonstrate an offeror's relevant past performance and experience qualifications for the contract award evaluation.
The form serves as an appendix to the Screening Information Request (SIR) and functions as a standardized template for capturing past experience details rather than prescribing specific products or services. Offerors are required to complete this form to substantiate their capability to perform under the five-year IDIQ contract for toner and printer consumables (750+ items). The form supports the FAA's Lowest Priced, Technically Acceptable (LPTA) evaluation methodology by allowing the government to assess offeror qualifications and historical performance relevant to this small business set-aside requirement under NAICS 325992. Submission of this completed form is mandatory for offerors seeking consideration for the single-award contract with a base year plus four one-year ordering periods.
