About this file
This document contains an exhibit for a past performance relevancy survey related to a federal solicitation for rebalancing an HVAC system. Offerors are asked to provide details on relevant past contracts they have held as a prime contractor or subcontractor, including the contract number, value, completion dates, and a description of the work performed and its relevance to the current solicitation. Three levels of relevance are defined: very relevant, relevant, and somewhat relevant. For each past contract, offerors must provide a point of contact from the owner or government agency. Additional information requested includes any liquidated damages assessed on contracts in the last three years, problems encountered and corrective actions taken, and any industry awards or unique skills. The related federal contract opportunity is for rebalancing an HVAC system issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 16. Offerors must complete and submit the past performance relevancy survey to be considered for the award.
