About this file
This is a Cost Proposal and Scoring Guide attachment for RFP #31786-00187 issued by the State of Tennessee, Department of Finance and Administration, Division of Benefits Administration, seeking administrative services for The Tennessee Plan, a self-insured supplemental medical insurance program for Medicare-eligible state retirees. Respondents are required to submit proposed monthly administrative fees on a per-member basis for each calendar year from 2027 through 2031, with the cost proposal remaining valid for at least 120 days following the proposal opening date. The evaluation methodology allocates a maximum of 50 points to the cost proposal component, calculated using a comparative formula that awards points based on the relationship between each proposal's evaluation cost and the lowest evaluation cost submitted across all proposals. The contract period runs from January 1, 2027, through December 31, 2031, with an evaluation factor of 48 members applied to the annual administrative fees for scoring purposes.
All monetary amounts must be submitted in U.S. currency with exactly two decimal places, and respondents must complete all cells in the pricing schedule without exception; blank cells or non-compliant entries will be treated as zero cost to the State. The cost proposal must be signed by an individual with authority to bind the respondent organization, with supporting documentation of signatory authority required if the signer is not the President or Chief Executive Officer. If all proposals receive evaluation costs totaling zero, the maximum section score will be awarded to all respondents, with the State using one cent ($0.01) as the baseline for calculating comparative point allocations. The current incumbent is UMR, Inc., with the existing contract expiring on June 30, 2028; the plan currently serves approximately 48,059 members as of July 2025, with historical enrollment growth of 1,500 to 2,000 members annually and annual claims costs increasing at approximately four percent.

| NOTICE: THIS COST PROPOSAL MUST BE COMPLETED EXACTLY AS REQUIRED | |
| COST PROPOSAL SCHEDULE— The Cost Proposal, detailed in this spreadsheet, shall indicate the proposed price for goods or services defined in the Scope of Services of the RFP 31786-00187 Attachment 6.6., Pro Forma Contract, for the entire contract period. The Cost Proposal shall remain valid for at least one hundred twenty (120) days subsequent to the date of the Cost Proposal opening and thereafter in accordance with any contract resulting from this RFP. All monetary amounts shall be numerical in U.S. currency and limited to two (2) places to the right of the decimal point. |
Refer to RFP 31786-00187 Attachment 6.6, Pro Forma Contract, Section A.2. for definition of Member.
Provide in tab "A. Admin Fee" your Proposed Monthly Administrative Fee per Member for calendar years 2027, 2028, 2029, 2030, and 2031.
This pricing schedule must be completed in full. Do not skip any cells. If the Respondent fails to detail all cost information for the services proposed as required, the State may determine the proposal to be non-responsive and reject it. Enter numeric figures only. Anything else, including blank cells or any deviated text, will be considered as a zero (0) and will be a cost of zero (0) to the State. The State cannot accept "N/A".
If the sum of the Evaluation Cost on a table equals zero, the Respondent will receive the maximum section score in the table. In this scenario, the State will use one cent (.01) in the formula listed in the table as the 'lowest evaluation cost amount from all proposals" to calculate all other proposals' point allocations
| NOTICE: |
| The Evaluation Factor associated with each compensable unit is for evaluation purposes only. The evaluation factors do NOT and should NOT be construed as any type of volume guarantee or minimum purchase quantity. The evaluation factors shall NOT create rights, interests, or claims of entitlement in the Respondent. |
This Cost Proposal must be signed, in the space below, by an individual empowered to bind the Respondent to the provisions of this RFP and any contract awarded pursuant to this RFP. If the individual signing this Cost Proposal is not the President or Chief Executive Officer, the Respondent must attach evidence to the Cost Proposal showing the individual’s authority to legally bind the Respondent.
