About this file
This is a Cost Proposal and Scoring Guide attachment (RFP Attachment 6.3) for RFP #31786-00187 issued by the State of Tennessee, Department of Finance and Administration, Division of Benefits Administration, seeking administrative services for The Tennessee Plan, a self-insured supplemental medical insurance program for Medicare-eligible retirees. Respondents must complete the cost proposal spreadsheet by providing proposed monthly administrative fees per member for calendar years 2027 through 2031. The cost proposal must be fully detailed with all cells completed using only numerical figures in U.S. currency with two decimal places; blank cells or non-compliant entries will be treated as zero. The proposal must be signed by an individual authorized to bind the respondent to the RFP and any resulting contract, with evidence of authority required if the signatory is not the President or Chief Executive Officer. The cost proposal will remain valid for at least 120 days following the proposal opening date.
The cost proposal scoring methodology allocates a maximum of 50 points based on evaluation cost, calculated using an evaluation factor of 48 applied to the annual administrative fee amount. The scoring formula divides the lowest evaluation cost amount from all proposals by the evaluation cost amount being evaluated, multiplied by 50 to produce the final cost score. If the evaluation cost equals zero across all proposals, the state will use $0.01 as the lowest cost benchmark for comparative scoring. The evaluation factors used for scoring purposes do not constitute volume guarantees, minimum purchase quantities, or any entitlement rights for respondents. The contract term runs from January 1, 2027, through December 31, 2031, with compensation structured as a monthly per-member-per-month (PMPM) rate based on the membership base of approximately 48,059 members as of July 2025.

| NOTICE: THIS COST PROPOSAL MUST BE COMPLETED EXACTLY AS REQUIRED | |
| COST PROPOSAL SCHEDULE— The Cost Proposal, detailed in this spreadsheet, shall indicate the proposed price for goods or services defined in the Scope of Services of the RFP 31786-00187 Attachment 6.6., Pro Forma Contract, for the entire contract period. The Cost Proposal shall remain valid for at least one hundred twenty (120) days subsequent to the date of the Cost Proposal opening and thereafter in accordance with any contract resulting from this RFP. All monetary amounts shall be numerical in U.S. currency and limited to two (2) places to the right of the decimal point. |
Refer to RFP 31786-00187 Attachment 6.6, Pro Forma Contract, Section A.2. for definition of Member.
Provide in tab "A. Admin Fee" your Proposed Monthly Administrative Fee per Member for calendar years 2027, 2028, 2029, 2030, and 2031.
This pricing schedule must be completed in full. Do not skip any cells. If the Respondent fails to detail all cost information for the services proposed as required, the State may determine the proposal to be non-responsive and reject it. Enter numeric figures only. Anything else, including blank cells or any deviated text, will be considered as a zero (0) and will be a cost of zero (0) to the State. The State cannot accept "N/A".
If the sum of the Evaluation Cost on a table equals zero, the Respondent will receive the maximum section score in the table. In this scenario, the State will use one cent (.01) in the formula listed in the table as the 'lowest evaluation cost amount from all proposals" to calculate all other proposals' point allocations
| NOTICE: |
| The Evaluation Factor associated with each compensable unit is for evaluation purposes only. The evaluation factors do NOT and should NOT be construed as any type of volume guarantee or minimum purchase quantity. The evaluation factors shall NOT create rights, interests, or claims of entitlement in the Respondent. |
This Cost Proposal must be signed, in the space below, by an individual empowered to bind the Respondent to the provisions of this RFP and any contract awarded pursuant to this RFP. If the individual signing this Cost Proposal is not the President or Chief Executive Officer, the Respondent must attach evidence to the Cost Proposal showing the individual’s authority to legally bind the Respondent.
