About this file
This document is a Public Contract Code Disclosure form required by the State of California for bidders on public works contracts. The form applies to the City of Pasadena's solicitation for construction management services on the Holly Street Bridge Seismic Retrofit and Rehabilitation Project, which is federally funded through the California Department of Transportation's Highway Bridge Program. Bidders must declare their compliance status regarding criminal convictions, disqualifications from previous government projects, and contempt of court findings related to labor law violations. The disclosure requires bidders to certify under penalty of perjury that they have not been convicted within the preceding three years of offenses including fraud, bribery, collusion, conspiracy, or antitrust violations in connection with public works contracts, and that they have not been disqualified from bidding on federal, state, or local government projects due to law or safety regulation violations.
The certifications required in this disclosure form are mandatory components of the bid submission process and apply to the bidder entity and all partners, members, officers, directors, and responsible managing employees. False certifications may subject the certifier to criminal prosecution. Additionally, bidders must confirm compliance with Public Contract Code Section 10232 regarding federal court contempt findings related to National Labor Relations Board compliance orders. These requirements ensure contractor integrity and eligibility to perform on the federally funded bridge rehabilitation project, which will involve on-site inspection services and construction materials testing under California and federal requirements.
