NEPA ENVIRONMENTAL CHECKLIST - SUBASENLON
NEW FORM EFFECTIVE 1/1/2013. This NEPA Environmental Checklist (EC) is utilized to determine the environmental rrequirements associated with a proposed project. Complete the first 2 pages of the form and attach a site map and a SOW or 1391. Then forward it to the PW Environmental Division. The Environmental Division will respond as soon as possible from receipt of a complete checklist and attachments. Type responses in the boxes/space provided. PLEASE NOTE: The Environmental review provided is only valid for 1 year.
