1.0 CONTAINMENT INTEGRITY
1.1 Tank containment (including tank top) contains water, debris, cracks, or fire hazard.
1.2 Secondary containment drain valve is inoperable, unsecured, or left in open position.
2.0 LEAK DETECTION
2.1 Visible signs of leakage from tank or during filling.
2.2 Visible signs of leakage onto surrounding area.
2.3 Signs of leakage into interstice of double‐walled tank
(visual or monitored).
2.4 Water in Primary Tank.
3.0 TANK EQUIPMENT
3.1 Tank fill box contains fuel, water, or debris.
3.2 Liquid level sensing devices are damaged.
3.3 Liquid level sensing devices are not easily readable at fill‐ port.
3.4 Pipe connections have leaks, corrosion, or damage.
3.5 High Level Alarm does not test OK. Test button may be on
tank, generator panel, or in remote fill‐port.
4.0 TANK ATTACHMENTS
4.1 Ladder or platform structures are corroding, deteriorating, or damaged.
4.2 Emergency vent is corroding, deteriorating, damaged, or
missing.
5.0 SPILL RESPONSE KITS
5.1 Spill kit is missing or not within reasonable distance to tank.
5.2 Spill kit container is in unsatisfactory condition (e.g., lid missing, holes/cracks, contains water).
5.3 Spill kit is not sealed (if applicable) or is in need of
restocking.
6.0 OTHER CONDITIONS
6.1 Other conditions exist that should be addressed for continued safe operation or that may affect the site spill prevention plan.

1.0 TANK STRUCTURE
1.1 Containment structure (outer shell) degraded, corroded, leaking, or otherwise compromised.
1.2 Evidence of paint or coating failure on tank exterior.
1.3 Tank shell is dented, buckled, bulging, corroded, or
cracked.
1.4 Tank top has low points or standing liquid.
2.0 TANK FOUNDATION AND SUPPORTS
2.1 Foundation supporting tank is settling or washed out.
2.2 Stormwater is unable to drain away from tank.
2.3 Grounding strap is unsecured or in poor condition.
3.0 TANK EQUIPMENT
3.1 Vents or components (primary and emergency vents) are
obstructed or not functioning properly.
3.2 Interstitial leak detection equipment is not legible or
functional.
3.3 Spill containment box is compromised through corrosion, debris, or leakage.
3.4 Spill containment box drainage valve is not operable and
closed.
3.5 Liquid level gauge does not operate correctly.
4.0 MISCELLANEOUS
4.1 Electrical wiring for control boxes/lights are in poor
condition.
4.2 Tank labeling is unreadable, or missing.
5.0 OTHER CONDITIONS
5.1 Other conditions exist that should be addressed for continued safe operation or that may affect the site spill prevention plan.
