Addendum to FAR 52.212-1, Instructions to O erors SAF/FM Quality Assurance (QA) Program Support

1. All communications (questions, proposals, etc.) shall be submitted via email to the Contracting O icer and Specialist:
a. Deanna Kerkho at deanna.kerkho .1@us.af.mil
b. Brittney Devallon at Brittney.devallon@us.af.mil
2. Proposals shall consist of 3 separate parts:
a. Pricing Volume
b. Technical Capability Volume
c. Past Performance Volume
3. Pricing Volume (completed on the SF1449)
a. Terms & Conditions – O erors are instructed to complete blocks 12, 17a and 30a, b, and c of the SF1449. In doing so, the quoter accedes to the contract terms and conditions as written in the solicitation, with attachments. The solicitation constitutes the model contract.
b. Pricing – Insert proposed unit and extended prices in the Pricing Schedule for all Contract Line Items (CLINs) on the SF1449.
c. Representations & Certifications – Complete the necessary fill-ins and certifications in the Contract Clauses and Provisions section of the SF1449, including FAR 52.212-3, O eror Representations and Certifications—Commercial Products and Commercial Services.
d. Notice of Evaluation – Note that FAR 52.217-8 will be included in the price evaluation by adding 50% of the value of the CLINs in Option Year 3 (CLINs 3002 and 3003) to the total price. This amount is for evaluation purposes only and will not be included in the awarded contract value.
4. Technical Capability Volume (10 pages max, 11pt font max) – This program is highly dependent on the contractor’s depth of knowledge and experience working with DAF FM processes, paired with web-based analytic site creation skills. Therefore, the Technical Capability Volume shall be specific, detailed, and complete to clearly and fully demonstrate that the o eror has a thorough understanding of the requirements for the associated e ort. The validity and thoroughness of the o eror’s Technical Capability Volume will be evaluated as evidence of the o eror’s understanding and capability to perform this requirement. Broad statements that the o eror understands, can or will comply with the PWS; or statements paraphrasing the PWS or parts thereof will be considered unacceptable. This volume shall contain no references to cost or pricing. The Technical Capability submission shall include, at a minimum, the following:
a. Program Management and Sta ing Plan – This shall include, at a minimum:
i. A detailed management and sta ing approach that illustrates an adequate plan to manage personnel throughout the life of the contract to ensure performance is accomplished IAW the PWS.
ii. An organizational structure, with descriptions of proposed roles and responsibilities
iii. A sta ing plan that addresses the labor mix of personnel currently employed and those projected for the requirement at hand
iv. A description of o eror’s recruitment and retention strategy to ensure employment of an adequate labor mix at all times. This includes maintaining the necessary quantity of qualified individuals with the requisite skillsets and experience at the appropriate level.
b. Transition Plan – O erors shall provide a Transition Plan to illustrate in detail how they will mobilize for contract performance, ensuring no degradation in service from the previous contract. O erors are instructed to propose a 45-day transition period at the start of the contract, during which time employee turnover and training with the incumbent contractor will commence. The Transition Plan shall include, at a minimum:
i. Gantt chart (or similar) outlining key mobilization activities, including a plan to a) recruit, b) train, and c) qualify and/or certify personnel during the 45-day transition period that will result in a 100% sta ed and qualified contractor workforce fully performing on the CLIN 0002 start date.
ii. If o erors do not believe a 45-day transition is necessary, submit instead a detailed rationale explaining why this is. The Government does not have resources to train new employees to perform these services at this time. Therefore, o erors’ rationale for decreased or excluded transition period should include their specific strategy for ensuring no gap in performance at the end of the current contract.
5. Past Performance Volume (5 references max; each 1 page max, plus CPARS if available) – O erors are instructed to provide information ONLY for recent and relevant past performance accomplished on Government and commercial contracts. Accordingly, only information regarding contract performance within 3 years and similar in scope and magnitude to this e ort will be considered.
a. Past Performance Volume – Submit a list of no more than 5 recent and relevant contracts performed for Federal agencies or commercial customers that demonstrates the ability to successfully perform the scope and breadth of the requirements described in this solicitation’s PWS. The Past Performance Volume must contain the following information for each contract listed:
i. Company/Organization Name:

iii. Contracting Agency/Customer:
iv. Contract Number:
v. Contract Dollar Value:
vi. Period of Performance:
vii. Current Customer POC (Contracting O icer if available) Name & Email:
viii. Description of compliance with contract terms & conditions:
ix. Comments regarding any known unacceptable performance reported, and/or corrective actions taken:
x. Explanation of how the scope of the referenced contract is similar to the work in this
PWS:
xi. Associated quality/satisfaction ratings, including CPARS if available (attach actual CPARS report, which does not count towards page limit):
b. Past Performance Questionnaires (PPQ) – O erors shall send PPQs, Attachment 3, to each of the Customer POCs listed in their Past Performance Volume, requesting that the Customer POCs complete and return the PPQ directly to the Government at deanna.kerkho .1@us.af.mil and Brittney.devallon@us.af.mil. It is the o eror’s sole responsibility to track the submission of PPQs and ensure the Government receives them not later than the due date and time established in this solicitation for o er submission.
c. No Past Performance – O erors with no recent and relevant past performance experience must submit a statement identifying this in their proposal submission.
d. Additional Sources of Information – The Government reserves the right to consider information regarding vendor performance from other sources not provided as part of the o eror’s proposal. However, o erors will be a orded an opportunity to respond to any derogatory information not submitted as part of the Past Performance Volume or PPQs.
