RE: RSQ 10-22 Continuing Professional Services ADDENDUM NO. 01
To prospective vendors:
The Utilities Commission is issuing the following addendum. As such it becomes an integral part of the proposal and must be acknowledged by the return of this signed form, with your proposal form, acknowledging receipt of the addendum.
The following questions were asked and are answered below.
1) Our company provides both Electrical Engineering and Civil Engineering services. Do we submit a qualification package for each service/discipline or in one package?
Yes, separate submittals.
2) On page 5 of the RSQ, it states that ‘Submittals’ should contain no more than fifty (50) double sided pages” Can you please clarify whether this is referring to 25 sheets of paper with 50 total pages or 50 sheets of paper with
100 total pages?
50 Sheets of paper which may be double sided.

3) Are you folks asking that respondents submit an SF330 for this solicitation?
No. Please follow the directions contained within the solicitation to respond.
4) In the proposal subjects’ section, the details/descriptions of 1
(Qualifications) and 7 (Past Project Description) are very similar, almost identical in terms of what is being asked. Can you please clarify the differences between these two sections?
1 (Qualifications): overview of consultant’s qualifications and capabilities, what makes them standout, and why they would be an asset to the UCNSB.
7 (Past Project Descriptions): detailed examples of past projects performed, who they performed them for, and the benefit the project provided to the client.
5) The 1. “Qualifications” and “7. Past Project Performance” sections (refer to pages 4 and 5 of the RSQ both request a list of projects completed in the past 10 years. For our firm, most of the projects we would list are the same projects that would apply to both sections. Outside of the requirement to include “details on the project the positive impact it had for the municipality” requirement for the “Past Project Performance” section, the requested details are generally the same for both sections and our responses would be very similar. Our questions are below: Please clarify the differences between the “projects” requirements for the two sections. Is it

10) Qualifications (#1) and Past Project Performance (#7) appears to be one in the same with the exception of #7 being focused on municipalities Do project references/examples need to be only focused on municipality related projects: If you sum the weighting of these two items it’s pretty heavy, so would be good to get some clarification on the difference of the two.
1 (Qualifications): overview of consultant’s qualifications and capabilities, what makes them standout, and why they would be an asset to the UCNSB.
7 (Past Project Descriptions): detailed examples of past projects performed, who they performed them for, and the benefit the project provided to the client.

SCOPE OF SERVICES
ETM Survey recognizes that the purpose of this contract is to provide professional surveying and mapping services on an as-needed basis for the City of New Smyrna Beach Utilities Commission that is compliant with State and City requirements.
APPROACH TO PROJECT
From an administrative standpoint, it is our goal to submit a superior survey product that is cost efficient and surpasses our the Commission’s expectations. The first step in the successful execution of an assigned task is a function of communication. ETM Survey’s Project Manager for this solicitation, Scott A. Graham, PSM will meet with the Utilities Commission’s Project Manager to determine the exact nature of the scope of services required for each specific survey project. A man hour assessment of the anticipated work effort will be produced based on a review of the specific site and analysis of past projects of a similar scope and magnitude. ETM Survey will then seek to negotiate a project specific contract with the Utilities Commission’s Project Manager that is fair and equitable to both parties.
Once the specific goals for the project have been clearly determined, Mr.
Graham will assemble the team best suited to that end. The project will be further assessed, and a specific plan will be formulated. Discussion with, and input from, all team members including Project Surveyors, CADD Technicians, Utility Coordinators, Field Crew Coordinators, and Field Crew Chiefs will be considered in this process. Additional meetings over the course of the project will keep Mr. Graham abreast of the progress and quality of the work effort. Mr. Graham will review and bear responsibility for all survey decisions required over the life of the project and will apprise the Utilities Commission’s Project Manager of the project status on a monthly basis or upon request.
2. CONSULTANT’S UNDERSTANDING
ETM | Page 14
Upon determination of the appropriate team, research of the public records and compilation of existing data pertinent to the project will be undertaken by the Project Surveyor, who supplies information to the CADD Technicians and oversees the production of preliminary maps and search ties for field crews. Field Crew Coordinators and Utility Coordinators are also involved at this juncture, aiding in our efforts to provide field crews with the information they need to optimize data collection efficiently. ETM Survey believes that a prepared and well-informed field crew is critical to the success of a project from the onset. We have extensive experience with many large scale projects involving Subsurface Utility Engineering (SUE) on contracts with JEA and many transportation projects throughout Florida.
As field work progresses, it is imperative that the work is monitored for accuracy and completeness on a regular basis. The work is usually broken up into logical segments and processed in increments. In this way, we are able to evaluate the data and determine if adjustments to field methodology are needed. This also allows our office personnel to begin mapping efforts prior to the completion of field work. Invariably, this work segmentation will also keep Mr. Graham involved with the project on a daily basis as decisions regarding interpretation of the data are required and reviewed.
ETM | Page 15
As the final mapping stage of production nears completion, the Project Surveyor begins the Quality Assurance (QA) and Quality Control (QC) review of the product. The product is reviewed for compliance with Chapters 177 and 472 of the Florida Statutes, Chapter 5J-17 of the Florida Administrative Code and with specific regard to any Utilities Commission ordinances and standards. After being reviewed by the Project Surveyor, the maps are then submitted to Mr.
Graham for a final review prior to being submitted to the Utilities Commission.
At ETM Survey, we believe that effective communication, coordination/planning, and a high standard of care are the most important attributes to a successful project. To ensure the accuracy and completeness of all project deliverables, ETM Survey has developed a project approach and QA/QC plan, proven successful, and followed for all projects. We have found that there is no substitute for a solid process or the timely application of proven skills and experiences throughout.
Our success is not measured by the age of our firm, but by our decades of successful project experience.
ETM | Page 16

An internal Quality Assurance/Quality Control program is used as a daily process at every level of service to ensure tasks are completed in accordance with proper accuracy and procedures. The primary goal of our Quality Assurance Program (QAP) is to assess whether our surveying services are meeting the Commission’s expectations and contract requirements while also initiating processes to improve team services. With direct input from Commission staff, the ETM Survey team will incorporate the following initiatives in a program that manages project performance and ensures high quality deliverables:
• Divide work efforts among staff according to their individual strengths.
