Summary of Findings
The summary condition of the bridge is FAIR.
Two exterior timber piles at the east abutment and one timber pile at the southeast wingwall are heavily rotted. Many deck planks are heavily rotted. One is broken on the north side in the western 1/3. Object markers are not in place.
Sections of the bridge railing are incorrectly lapped on the south rail and there is a gap between rail sections at ends of the 7th span from the east. Vegetation is encroaching into the roadway near the ends of the bridge. The shoulders at the west end of the bridge are sloughing due to lack of adequate retention by the wingwalls. There is a minor amount of sand and gravel on the deck at the ends of the bridge that causes abrasion and wear of the timber planks. There are split and loose runner planks and one is broken on the north side in the western 1/3. Pile bracing members are split, broken and missing in several places. There is submerged debris in the channel on both sides of the bridge that may become caught at the bridge.

Work Recommendation Remarks
1/Recommended work are items noted by the inspection team on the date of inspection. Urgent items are those which pose a current threat to the strength or stability of the bridge, or involve a significant safety concern. Items are listed in their order of importance. Item 1 considered most important. Adjustments for location include geographical region and remoteness of the bridge adjustment. For multi-use structures such as water control structures or dam spillways, repair or reconstruction estimates, if applicable, are for the bridge component only.
4/Priority assessment value is a representation of the relative priority of a recommendation, with a value of 1 representing the highest priority and 7 representing the lowest priority.
3/Items d. and e., when needed, are used for replacements, and work items over $100,000 in construction value, and use a sliding scale based on the construction estimate. If the item is accomplished by MMS, the mark-up should be adjusted accordingly.
2/Item c., when needed, is used for items up to $100,000 in construction value, except replacements, and includes a mark-up of approximately 18% assuming MMS program accomplishment. If the item is accomplished by other than MMS, the mark-up should be adjusted accordingly.

RATING VERIFIED BY: Paul Jordan, 8/12/16
One lane bridge signs are in place. Object markers are not in place.
Sign Status
The load rating from 1996 is still considered valid. No significant changes in the condition of the bridge which would affect the rating were observed during the 2016 inspection. The load capacity and weight limit posting recommendations remain as shown above.
Sign Recommendation Remarks
Important Note Regarding Posting Recommendations levels to which a structure may safely be subjected for limited passages of the loads. Should the need arise to subject the bridge to sustained loading at or near this level, the Project Leader should contact the Regional Engineer for guidance regarding an appropriate safe load capacity.

Description of Bridge
The bridge was constructed in approximately 1970 and the removable steel main span added in 1980. The bridge consists of one main span of steel stringers and 18 approach spans of treated timber stringers, all with treated timber plank deck and runner planks. The spans are all supported on treated timber pile bents and abutments. Additional work, including piling under the utility boxes and at the 2nd bent from the west, was done in 1991. Also, at some time in the past, intermediate supports were added which are now bents 10 and 12 (abutment 1 is at east).
Comparison to Previous Inspection
Next Inspection
Additional Comments
There are no significant changes is the condition of the bridge since the previous inspection. There is a broken runner plank and deck plank on the north side in the western 1/3 of the bridge.
The next inspection for this bridge is recommended as a routine inspection with a full report in two years.
Underwater inspections of the timber was completed July 27, 2016 and August 7, 2012. Findings are in a separate report, and are summarized in the condition ratings section of this report. There continues to be more deterioration of the piling above the water than below.
An under bridge inspection vehicle was used during the 2016 and 2012 inspections. It is recommended that the UBIV be used every four years.

8 - Very Good Condition - no problems noted.
7 - Good Condition - some minor problems.
6 - Satisfactory Condition - structural elements show some minor deterioration.
5 - Condition - all primary structural elements but may have minorFair section loss, cracking, spalling, or scour.
4 - Poor Condition - advanced section loss, deterioration, spalling, or scour.
3 - Serious Condition - loss of section, deterioration, spalling, or scour have seriously affected primary structural components. Local failures are possible.
Fatigue cracks in steel or shear cracks in concrete may be present.
2 - Critical Condition - advanced deterioration of primary structural elements.
Fatigue cracks in steel or shear cracks in concrete may be present or scour may have removed substructure support. Unless closely monitored it may be necessary to close the structure until corrective action can be taken.
1 - Imminent Failure Condition - major deterioration or section loss present in critical structural components or obvious vertical or horizontal movement affecting structure stability. Structure is closed to traffic but corrective action may return structure to light service.
0 - Failed Condition - out of service; beyond corrective action.

BENTS/PIERS
60.10 Bearing Seats 6 Steel and timber stringers bear on treated timber pile caps.
Treated timber - Minor to moderate checks on bottoms of most, minor rot.
Treated timber, a few supplemental piles driven in 1981.
Piles were sounded near mid-height and at ground.
Moderate to heavy rot and deep checks above water line in older piles. North and center piles not centered under cap at bent on west end of steel span, 33% bearing loss. South pile at Bent 13 from east had top 8' replaced and spliced with steel angles. Supplemental cap and pile support at heavily rotted south pile of Bent 18 from east. Center pile at Bent 18 is offset 1/3 of pile diameter under cap. Middle pile at east Bent is split from top down 10'. Middle pile at west Bent has deep checks. Steel cable is strung through the west portion of the bridge to supplement horizontal bracing, but is slack. Numerous horizontal braces have minor splits, and horizontal brace at 7th bent from west is failed.
Findings of the 8/7/12 underwater inspection indicate little deterioration, and north pile of 11th bent from west has 50% section loss at a pipe collar (previous repair?) from -10' to the mudline.
