About this file
Summary
This is a Conflict of Interest Questionnaire (Form CIQ) required under Texas Local Government Code Chapter 176 for vendors conducting business with local governmental entities. The form applies to the three state and local contract opportunities involving Mt. San Antonio College (civil engineering design services), Collin County (incarcerated people's communications services), and the City of Edinburg (drag strip maintenance and operations). Vendors must complete and file this questionnaire with the appropriate records administrator not later than the seventh business day after the later of: (1) the date the vendor begins discussions or negotiations to enter into a contract with the local governmental entity, submits an application, or submits a response to a request for proposals or bids; or (2) the date the vendor becomes aware of an employment or other business relationship with a local government officer, gift disclosures, or family relationships as defined by the statute. The form requires disclosure of employment or business relationships with local government officers or their family members that result in taxable income exceeding $2,500 during the 12-month period preceding contract awareness, gifts with aggregate value exceeding $100 provided to officers or family members within the same period, and any family relationships with local government officers.
Failure to file a required and accurate Conflict of Interest Questionnaire constitutes a misdemeanor offense under Texas Local Government Code Section 176.006. The questionnaire must be updated within seven business days if the originally filed form is discovered to be incomplete or inaccurate. Vendors should consult the complete text of Texas Local Government Code Chapter 176 for detailed definitions of "business relationship," which excludes transactions subject to rate or fee regulation, transactions conducted at prices and terms available to the public, and purchases or leases of goods or services from chartered entities subject to regular examination by regulatory agencies.
