About this file

This is a Conflict of Interest Questionnaire form provided by the Texas Ethics Commission for vendors conducting business with local governmental entities in Texas. The form implements requirements under Chapter 176 of the Local Government Code, as amended by H.B. 23 during the 84th Legislative Session.

Vendors must file the completed questionnaire with the records administrator of the local governmental entity no later than the seventh business day after the later of either: (1) the date the vendor begins discussions, negotiations, or submits an application or proposal related to a potential contract with the entity, or (2) the date the vendor becomes aware of a triggering conflict of interest. The questionnaire requires disclosure of employment or other business relationships with local government officers or their family members that result in taxable income exceeding $2,500 during the 12-month period preceding the vendor's awareness of a contract execution or consideration; gifts with an aggregate value exceeding $100 provided to local government officers or their family members during the same period; and any family relationships with local government officers. Vendors must identify the specific officer involved, describe the nature of any relationship, and certify whether the officer or family member is receiving or likely to receive taxable income from the vendor, whether the vendor is receiving taxable income from the officer or family member outside the governmental entity, and whether gifts have been provided. Failure to comply with filing requirements constitutes a misdemeanor offense under Texas law.
