About this file
Summary
This is a Task Order Response Form template developed by the Arizona Department of Forestry and Fire Management (DFFM) under the Office of the State Forester for the procurement of all-inclusive single engine air tankers (SEATs) and related services. The form establishes the procedural framework for contractors to respond to specific task orders issued under existing contracts with DFFM. Contractors must familiarize themselves with the task order, project area maps, contract terms, and all relevant supporting documents before submitting responses. The template requires contractors to provide current certificates of insurance and list all available aircraft, fuel support vehicles, pilots, and relief crew members with current OAS (Outdoor Airtanker Service) certifications. Responses must be submitted in standard formats including PDF, Microsoft Word, Excel, or PowerPoint documents. The contract structure supports two service models: Exclusive Use (EU) periods with dedicated aircraft availability and Call When Needed (CWN) periods with on-demand services, with provisions for supplemental aircraft to be added at DFFM's discretion.
The evaluation process prioritizes cost (maximum 45 percent), contractor planned start date alignment with DFFM's timeline (maximum 35 percent), and past contract performance history (maximum 20 percent), with the highest combined score determining the preferred contractor. Pricing shall be submitted for flight time, mobilization, extended standby periods, relief crew transportation, and miscellaneous expenses at cost with no markup. All completed treatment requires mandatory inspection by DFFM representatives prior to invoice approval. Invoices must include project documentation, transportation worksheets, AMD-23E forms, receipts for miscellaneous expenses, and must be submitted with specified billing information to designated DFFM representatives. The template allows DFFM to reduce the maximum number of contractors if prices exceed budgetary constraints or to reject all offers if deemed too costly. DFFM reserves the right to issue task order amendments and modifications during project implementation, though modifications require formal approval through the DFFM Task Order Amendment form.
