About this file
This is a Task Order Template document issued by the Arizona Department of Forestry and Fire Management (DFFM) under the Office of the State Forester, establishing the framework for soliciting contractor offers for landscape-scale fuels treatment planning and wildfire protection projects. The template provides a standardized format for task order issuance, including project name, location, performance period, scope of work, and deliverables specific to Single Engine Air Tankers (SEATs) and related services. Contractors must hold a current state contract corresponding to the scope of work and demonstrate compliance with all insurance requirements. The document specifies that responses must be submitted by the Response Due Date indicated on individual task orders, and includes provisions for project acceptance through scheduled inspections with the project forester prior to invoicing and final payment.
Evaluation of offers follows a weighted scoring methodology with Cost weighted at a maximum of 45 percent (lowest offer receives maximum score), Contractor Planned Start Date alignment at a maximum of 35 percent (with tiered scoring based on days relative to DFFM Planned Start Date), and Past Contract Performance Evaluation at a maximum of 20 percent (with contractors lacking prior evaluations receiving maximum score by default). Highest composite score determines award, with ties resolved by lowest offer. The template allows DFFM to reduce the maximum number of selected contractors if prices exceed budgetary constraints. Invoices must include specific documentation including completed Exhibits 4 and 5, transportation worksheets, receipts for miscellaneous expenses, and must be submitted within 30 days of service acceptance date. Contractor performance is evaluated using a standardized Contract Performance Evaluation form assessing adherence to bid specifications, timely commencement and completion, and compliance with contract specifications, with scores ranging from zero to 0.20 points maximum.

[Criteria listed in Order of Importance, criteria can be changed as needed for the Task Order]
a. Offers shall be evaluated using the following criteria:
1) Cost (lowest offer receives max score) (max 45%)
i. Scores are weighted based on the lowest offer
2) Contractor Planned Start Date aligns with/or is within specified time frames following the
DFFM Planned Start Date (earlier start date receives higher score) (max 35%)
i. 0-1 days past DFFM Planned Start Date (35%)
ii. 2-7 days past DFFM Planned Start Date (25%)
iii. 8-14 days past DFFM Planned Start Date (15%)
iv. If the DFFM Planned Start Date exceeds 14 days from Award Confirmation, it shall be assumed that the contractor will start implementation on the DFFM Planned Start Date and therefore will be awarded the max score 35%, by default.
3) Past Contract Performance Evaluation (max 20%)
i. The score from the most recent Contractor Performance Evaluation form
ii. If the contractor has not received a previous Contractor Performance Evaluation, the contractor will receive the max score 20%, by default.
4) Total Score
i. Highest score(s) determines the preferred contractor(s) for contract award.
ii. Any tying score results will default to lowest offer (higher Price Factor score) for award
iii. If the project will require the selection of more than one contractor (maximum number of contractors will be determined prior to TO release), the evaluator will determine the median score value.
⮚ DFFM shall reserve the right to reduce the maximum amount of contractors for offer if the prices are deemed too high, based on budgetary constraints.
