About this file
This is a Payment of Employee Healthcare Expenses Certification form issued by the State of Maryland under State Finance & Procurement Article Title 17, Subtitle 8, effective July 2024. The certification applies to bidders and subcontractors on state and local contract projects. Contractors must certify their compliance with employee healthcare expense requirements for employees working on or at project sites. The form requires selection of one of three compliance options: paying aggregate employee healthcare expenses equal to at least 5% of aggregate social security wages; paying 50% or more of the required premium for credible health insurance plan coverage; or qualifying for exemption as a firm with 30 or fewer employees.
Bidders and subcontractors must have an authorized representative sign and date the certification, providing company name, title, street address, and city/state/ZIP code. False information submitted on this certification subjects the responsible party to civil penalties ranging from $2,500 to $25,000 per violation under Maryland Annotated Code Section 17-804. This certification is a mandatory compliance requirement for participation in state and local contracting opportunities in Maryland and must be completed prior to bid submission or contract award.
