About this file
Attachment E is a Historical Small Business Utilization form that must be completed by all offerors—both large and small businesses—to provide supporting documentation for the evaluation of Factor 5 (Small Business Utilization and Participation) in response to Solicitation N40085-26-R-0020 for Design-Bid-Build P171 Weapons Magazines at Naval Weapons Station Yorktown, Virginia.
The form requires offerors to document small business subcontracting achievements and goals for up to five projects submitted under Factor 1 (Corporate Experience). For each project, offerors must provide: total project dollar value, total self-performed value, total subcontracted value, and a detailed breakdown of subcontracting by category including Small Business Concerns (SB), Large Business Concerns, Small Disadvantaged Business (SDB), Women-Owned Small Business (WOSB), HUBZone Small Business, Veteran-Owned Small Business (VOSB), and Service-Disabled Veteran-Owned Small Business (SDVOSB). All percentages must be rounded to the nearest tenth of a percent and calculated as a percentage of either total subcontract value or total contract value based on how goals were established. Offerors must provide written explanations for any unmet small business goals and for projects with zero small business subcontracting achievement; simply acknowledging missed goals is insufficient. The form must be completed exactly as provided without attachments, addendums, or alternative formats, though offerors may expand sections as needed for adequate response.
