About this file
This file is an attachment that provides a template for offerors to document and describe relevant past contract experience as part of their proposal submission for the Fort Hood Installation Custodial Services solicitation (W91151-26-R-A004).
The template requires offerors to submit information on up to three pages per relevant contract, including: contractor name, contract number, contract type, award date, period of performance, annual and total dollar values, total maintainable square feet serviced, and contact information for the Procuring Contracting Officer, Administrative Contracting Officer, and Contracting Officer's Representative. Offerors must provide a brief summary of work requirements performed under each referenced contract, followed by a written narrative explaining how the performance requirements are relevant to the instant requirement and demonstrating similarity in nature of work, complexity, and magnitude. The template also requires offerors to disclose any negative performance documentation, including cure notices, show cause letters, or contract discrepancy reports received, along with descriptions of corrective actions taken. Such negative performance documentation is not subject to the three-page limit per contract reference.
