About this file
This is a Reporting Requirements attachment for the Nebraska Medicaid Enrollment Broker contract issued by the State of Nebraska Department of Health and Human Services. Monthly reports due by the 15th of each month include Call Center Queue Statistics, Call Reasons, Call Center Quality Summaries, Enrollment Activity, Enrollment Methods, Primary Care Physician Selection data, Executive Dashboard summaries, Executive Operational Summaries, Health Plan Changes, For Cause requests, File Transfers, Mailed Materials, Returned Mail details, Website Availability and Response Time, EB Grievances, Health Plan Grievances, and Fraud, Waste and Abuse reports. Additionally, the EB must submit ad hoc reports within five business days of request from Medicaid and Long-Term Care, MLTC designees, the State Legislature, or Centers for Medicare & Medicaid Services, and quarterly reports on Open Enrollment Change Period activity due by April 15th.
Annual reporting requirements include an Open Enrollment Executive Summary Dashboard due January 15th, a fully executed Engagement Letter with a CPA due December 1st, an audit report due June 1st, a Systems Refresh Plan due January 31st, an independent security audit report due within 30 days of completion at the EB's expense, Ownership Disclosure information due January 31st per 42 CFR 455.100-455.106, and a Member Education Plan due November 30th. All reports must be submitted using MLTC-provided templates or in formats approved by MLTC, with due dates automatically extended to the next business day if they fall on weekends or state-recognized holidays. These requirements establish detailed accountability and performance monitoring mechanisms for the Enrollment Broker throughout the contract term.
