About this file

This is a contractor certification form that serves as Attachment 2 to RFQ No. 36C24924R0075 for Mobile CT Lung Cancer Screening Services at James H. Quillen and Lexington VAMC CBOCs. The certification requires contractors to comply with the Immigration and Nationality Act of 1952 (as amended) and related regulations enforced by Homeland Security, Immigration and Customs Enforcement, and the U.S. Department of Labor regarding non-immigrant foreign nationals working under the contract.

The document mandates that contractors cannot knowingly employ illegal aliens or foreign nationals who have violated their immigration status while performing services for the Department of Veterans Affairs. Contractors must comply with E-Verify requirements under Executive Order 12989 and applicable FAR regulations. Failure to comply may result in the prohibition of non-compliant foreign nationals from working on VA patient referral services and could lead to contract termination. The certification must be signed by the contractor, who must also obtain similar certifications from subcontractors, with false certifications subject to prosecution under 18 U.S.C. 1001.
