About this file
This file is a Past Performance Questionnaire template designed to evaluate contractors' performance on previous federal contracts within the past three years. The questionnaire consists of three parts: contract identification details to be filled out by the contractor, performance evaluation to be completed by the point of contact, and return information. The evaluation uses a simple two-tier rating system of "Acceptable" or "Unacceptable" across five key performance areas: compliance with specifications, project management effectiveness, timeliness of performance, cost control effectiveness (for cost reimbursement contracts only), and commitment to customer satisfaction.
This template is associated with solicitation W91ZLK-25-R-0002 for Facilities Infrastructure and Logistics Operations Support Services at Aberdeen Proving Ground, MD, supporting multiple commands including DEVCOM-CBC, DEVCOM-HQ, CECOM, and CMA. Proposals must be submitted via email by January 10, 2025 at 10am ET to specified contracting officers. The solicitation emphasizes that proposals should be complete as discussions are not planned, and only SAM-registered offerors will be considered.
