About this file
This is a Past Performance Questionnaire used by the Bureau of Engraving and Printing (BEP) to evaluate contractor performance for the Cooling Towers project/requirement.
The questionnaire requires offerors to submit references from at least three assessors who have provided relevant services within the last five years. Relevant services are defined as similar in size, scope, and complexity to BEP's cooling tower requirement. The offeror completes Section II with contract details, and the assessor(s) validate this information and complete Sections III and V. Assessors must email completed questionnaires directly to Laqulla.Williams@bep.gov no later than 12:00 PM Central Time on Friday, October 9, 2026. Section II requires information including contractor name and address, contract number, contract type, initial and final contract values, period of performance, project description, contracting role (prime or subcontractor), and termination history (convenience, default, or N/A). Section III captures assessor information including name, organization, title, phone/email, and the assessor's role in contract administration with their period of involvement. Section V contains a performance evaluation matrix assessing the contractor across Business Relations, Timeliness, Quality, and Cost categories using a six-point scale: Exceptional, Very Good, Satisfactory, Marginal, Unacceptable, and Not Applicable. The questionnaire concludes with a yes/no question regarding whether the assessor would award the contractor another contract, with space for explanation if the answer is no, and a general comments section requiring the assessor's signature and date.
