About this file
This document is an asbestos survey report for Structure #168B, a multi-family housing unit located on the Standing Rock Community Schools campus. The report identifies the presence of asbestos-containing materials (ACM) in the joint compound on ceilings and walls throughout the apartment, covering an estimated 3,248 square feet. The report estimates the cost of asbestos abatement at $16,240. The document also includes a radon survey, indicating radon levels within the unit are acceptable but higher levels were detected in the crawlspace. Additionally, the report identifies various regulated materials and miscellaneous hazardous materials, with an estimated $1,000 remediation cost. Finally, the report notes the presence of mold growth in the crawlspace and areas of the kitchen due to moisture issues.
This survey report is related to a federal contract opportunity for Environmental Remediation Services under Solicitation Number 140A1625R0007, issued by the Department of the Interior Bureau of Indian Affairs Central Office.

STANDING ROCK COMMUNITY SCHOOLS STRUCTURE #168B
1.0 BACKGROUND INFORMATION
Structure #168 was constructed in 1967 and is a two-story building with a dirt floor crawlspace. Unit B is a 1,388 ft 2 apartment. The structure serves as a multi-family housing unit.
The roofing system is flat with rolled roofing and black tar over wood and the exterior of the structure consists of a combination of brick, plywood and stucco.
The interior floor finishes include carpet over sheet vinyl flooring, and sheet vinyl flooring over wood; the interior wall finishes include smooth gypsum wallboard (GWB) with heavy wall texture on GWB in the bathroom only; and the interior ceiling finishes include smooth gypsum wallboard throughout the unit with heavy texture observed only in the bathroom.
LEGEND did not observe any pipes or pipe insulation on the main floor or second floor of the apartment unit. However, fiberglass pipe insulation with PVC elbows were observed in the crawl space. The residence was heated with electric radiator units.
Photo 59 depicts the exterior of structure #168B.
2.0 ASBESTOS SURVEY
A total of twenty (20) samples were collected from suspect asbestos-containing materials (ACM) from structure #168B. Laboratory analysis results indicate five
(5) of the samples tested positive for asbestos.
LEGEND surveyed and sampled the exterior of units 168-A through 168-D as part of unit (168A) report. Refer to unit 168-A for sample results and laboratory data.
Table #1 includes asbestos sampling data.

2.3 Estimated Asbestos Abatement Costs
LEGEND Technical Services, Inc. estimates abatement costs (removal and disposal) of RACM, CAT I, and CAT II in the various locations of the structure as follows:
ACM QUANTITY TOTAL
Joint Corn •ound 3,248 ft $16,240.00
Total Abatement Costs $16,240.00
3.0 RADON SURVEY
One (1) radon samples were collected from inside structure #168B.
The sample collected from the first floor living room detected a radon level of 0.6 pCi/I and the US EPA states that there is little short-term risk with test results in this range (0.6 to 1.9 pCi/L). However, because radon levels fluctuate daily as well as seasonally, you may want to retest during another season. Additionally, if you make any structural changes or start to use a lower level of the building more frequently, you should test again.
LEGEND collected a radon sample in the crawlspace at the time of the inspection, however, the sample results were not provided for this unit. Please consult the radon sample levels for units 168A, 168C and 168D. Data suggests that radon levels in the crawlspace exceed the 4.0 pCi/L EPA criteria but are acceptable within the unit.
Radon data follows this narrative.

4.1 Estimated Remediation Costs
LEGEND Technical Services, Inc. estimates remediation costs (removal and disposal) of the regulated materials and miscellaneous hazardous materials in the various locations of the facility to be $1,000.00.
5.0 MOLD SURVEY
The dirt floor crawlspace located in the living room closet smelled musty.
LEGEND observed mold growth on the fiberglass pipe insulation and water intrusions on the wood floor joists to the main floor. It appears the mold growth on the fiberglass pipes is caused by foundation seepage and high humidity levels in the crawlspace. Photo 60 depicts the mold growth on the fiberglass pipe insulation.
LEGEND observed two buckets full of water under the kitchen sink. It appears as there is a water leak from the kitchen sink/pipe.
LEGEND observed deteriorated GWB on the south wall and ceiling above the kitchen sink. The mold growth can be attributed to moisture generated by cooking in the kitchen and or leak from the second floor bathroom.
Black stains and heavy areas of cob webs are visible from the top of the stairwell landing. The stains and cob webs are on the tops of the walls and ceiling near the fan and appear to be from the lack of cleaning as the area of concern is up high and hard to reach.
