About this file
This document is a Statement of Work detailing the results of surveys conducted on a multi-unit residential building, Building 168, located on the Standing Rock Indian Reservation in Fort Yates, North Dakota. The surveys identified various hazards in the building including lead-based paint, asbestos-containing materials, high radon levels, and extensive mold growth. The document provides detailed information on the location, year built, square footage, and specific hazards found in each unit of the 4-plex building. It also includes recommendations for abatement and remediation of the identified issues. This Statement of Work appears to be related to a larger Environmental Remediation Services contract opportunity issued by the Department of the Interior Bureau of Indian Affairs.

HAZARDS REMARKS
LBP detected on stairway balusters in stairway. Abate and replace per SOW.
Asbestos Survey 6/32 samples tested positive. Remove and dispose of all ACM materials. Joint compound(ceilings and walls)/location: throughout the apartment (includes main level and upper level)
Radon Survey First floor living room detected 2.1pCi/I. Basement 9.1pCi/I. Basement level exceeds the EPA 4.0pCi/I guideline.
Mold Survey
Heavy mold growth was observed on the GWB walls and ceilings throughout the residence. Residence appears to have been vacant for some time with no power and heat. All finish materials within the unit had moisture damage. Water stains and mold growth was observed on the fiberglass pipe insulation in the dirt floor crawlspace and on the wood floor joists in the crawlspace.
LBP detected on stairway balusters in stairway. Abate and replace per SOW.
Asbestos Survey 5/20 samples tested positive. Remove and dispose of all ACM materials. Joint compound(ceilings and walls)/location: throughout the apartment (includes main level and upper level).
Radon Survey First floor living room detected 0.6pCi/I. A radon sample in the crawlspace was collected, however, the sample results are not provided for this unit.
Mold Survey
The dirt floor crawlspace located in the living room closet smelled musty. Mold growth observed on the fiberglass pipe insulation and water intrusions on the wood floor joists to the main floor appears to be from foundation seepage and high humidity levels in crawlspace. 2 buckets full of water located under the kitchen sink, it appears there is a leak from the kitchen sink/pipe. Deteriorated GWB observed on the South wall and ceiling above the kitchen sink can be attributed to moisture generated by cooking in kitchen or leak from 2nd floor bathroom. Black stains and heavy areas of cob webs are visible from the top of the stairwell landing due to lack of cleaning as area is high up and hard to reach.
LBP Survey No LBP detected
Asbestos Survey 8/35 samples tested positive. Remove and dispose of all ACM materials. Joint compound(ceilings and walls)/location: throughout the main floor of the house. Light liner/location: 2nd floor north bedroom.
Radon Survey First floor living room detected 0.9pCi/I. Basement 5.3pCi/I. Basement furnace room level exceeds the
EPA 4.0pCi/I guideline.

South wall and ceiling above the kitchen sink attributed to moisture generated by cooking in kitchen or leak from 2nd floor bathroom. Light mold growth was observed on the GWB walls and ceiling in the bathroom presumably due to condensation from lack of exhaust fan. Black stains and heavy areas of cob webs are visible from the top of the stairwell landing due to lack of cleaning as area is high up and hard to reach.
LBP Survey No LBP detected
Asbestos Survey 6/40 samples tested positive. Remove and dispose of all ACM materials. Joint compound(ceilings and walls)/location: throughout the apartment (includes main level and upper level).
Radon Survey First floor living room detected 0.6pCi/I. Basement 8.4pCi/I. Basement level exceeds the EPA 4.0pCi/I guideline.
Mold Survey
The dirt floor crawlspace located in the living room closet smelled musty. Mold growth observed on the fiberglass pipe insulation and water intrusion on the wood floor joists to the main floor appears to be from foundation seepage and high humidity levels in crawlspace. The dirt floor in the crawlspace was also noted to be wet. Light mold growth was observed on the GWB walls near the bathtub and
GWB ceiling in the bathroom presumably due to condensation from lack of exhaust fan. Black stains and heavy areas of cob webs are visible from the top of the stairwell landing due to lack of cleaning as area is high up and hard to reach. Deteriorated GWB walls observed under the windows in the second floor bedrooms appears to be from old or faulty windows and condensation from windows in the winter.
