STANDING ROCK COMMUNITY SCHOOLS STRUCTURE #168C
1.0 BACKGROUND INFORMATION
Structure #168 was constructed in 1967 is a two-story building with a dirt floor crawlspace. Unit C is a 1,388 ft 2 apartment. The structure serves as a multi-family housing unit.
The roofing system is flat with rolled roofing and black tar over wood and the exterior of the structure consists of a combination of brick, plywood and stucco.
The interior floor finishes include carpet over sheet vinyl flooring, and sheet vinyl flooring over wood; the interior wall finishes include smooth gypsum wallboard (GWB); and the interior ceiling finishes include smooth gypsum wallboard.
LEGEND did not observe any pipes or pipe insulation on the main floor or second floor of the apartment unit. However, fiberglass pipe insulation with PVC elbows were observed in the crawl space. The light in the north bedroom of the second floor was observed to have a fibrous light liner. The residence was heated with electric radiator units.

2.0 ASBESTOS SURVEY
A total of thirty-five (35) samples were collected from suspect asbestos-containing materials (ACM) from structure #168C Laboratory analysis results indicate eight (8) of the samples tested positive for asbestos.
LEGEND surveyed and sampled the exterior of units 168-A and 168-D with unit (168A). Refer to unit 168-A survey report for sample results and laboratory data.
Table #1 includes asbestos sampling data.

2.3 Estimated Asbestos Abatement Costs

3.0 RADON SURVEY
Two (2) radon samples were collected from inside structure #168C.
The sample collected from the first floor living room detected a radon level of 0.9 pCi/I and the US EPA states that there is little short-term risk with test results in this range (0.6 to 1.9 pCi/L). However, because radon levels fluctuate daily as well as seasonally, you may want to retest during another season. Additionally, if you make any structural changes or start to use a lower level of the building more frequently, you should test again.
The sample collected from the basement furnace room detected a radon level of
5.3 pCi/I. The US EPA states that for test results in this range you should conduct either a short- or long-term follow-up measurement. If, however, this is a follow-up (confirming) test, it is recommended that you take remedial action to reduce these radon levels.
Table #2 includes radon data.
4.0 REGULATED MATERIALS SURVEY

4.1 Estimated Remediation Costs
LEGEND Technical Services, Inc. estimates remediation costs (removal and disposal) of the regulated materials and miscellaneous hazardous materials in the various locations of the facility to be $1,000.00.
5.0 MOLD SURVEY
The dirt floor crawlspace located in the living room closet smelled musty.
LEGEND observed mold growth on the fiberglass pipe insulation and water intrusions on the wood floor joists to the main floor. It appears the mold growth on the fiberglass pipes is caused by foundation seepage and high humidity levels in the crawlspace. Photo 62 depicts the mold growth on the fiberglass pipe insulation.
LEGEND observed deteriorated GWB on the south wall and ceiling above the kitchen sink. The mold growth can be attributed to cooking with high condensation levels in the kitchen from cooking and or leak from the second floor bathroom.
Light mold growth was observed on the GWB walls and ceiling in the bathroom.
LEGEND did not observe an exhaust fan in the bathroom and the mold can be attributed to the lack of an exhaust fan and condensation on the walls and ceiling. LEGEND also observed a large plywood sheet screwed into the ceiling indicating prior mold or water problems.
Black stains and heavy areas of cob webs are visible from the top of the stairwell landing. The stains and cob webs are on the tops of the walls and ceiling near the fan and appear to be from the lack of cleaning as the area of concern is up high and hard to reach.
