About this file
This document is a PACAF Regional Support Center Site Arrival Request (SAR) form used for coordinating travel to various Air Force locations. The comprehensive five-page form requires detailed information about travelers, travel logistics, and specific site access requirements. Key details include submission timelines (14 calendar days for U.S. citizens, 45 days for foreign nationals), mandatory pre-travel preparations such as emergency medical evacuation plans, and strict guidelines for equipment, photography, and secure area access. Travelers must provide personal information, travel dates, purpose of visit, means of transportation, and potential additional support needs like equipment, transportation, or special permits.
The form covers multiple sections including requestor information, travel details, aircraft information (if applicable), additional requirements, traveler information (up to 20 travelers), and a traveler informational checklist. Notable restrictions include prohibitions on Bluetooth-capable devices in restricted areas, requirements for base passes, and mandatory documentation like passports for foreign nationals. The form must be submitted to specific email addresses based on the destination, with approval provided by the 611th Air Support Squadron. Travelers are responsible for their own medical evacuation plans and must adhere to strict baggage and equipment guidelines specific to their destination.
