About this file

This document constitutes financial proposal instructions and compliance requirements for a Preferred Provider Organization, Self-Funded (PPO-SF) medical plan submitted to the Maryland Transit Administration (MTA) under Solicitation No. AGY-24-026-SR-A, Amendment 11. Offerors must complete multiple financial exhibits including administration and network access fees, claims repricing analysis, and financial proposal summaries for a contract covering five base years plus two three-year renewal options. The proposal requires detailed submissions of de-identified aggregate claims and enrollment data provided in accompanying Excel files, with offerors obligated to reprice medical claims based on their current network provider contractual arrangements as of November 1, 2025, and return detailed medical claim files and provider match files with specified additional data fields appended to demonstrate network participation and repricing methodologies.

Offerors must quote all administration and network access fees on a fully-loaded, mature basis per employee per month (PEPM), guaranteed regardless of actual enrollment, with fees held flat for the first three years and annual increases not to exceed four percent thereafter. The current uniform enrollment assumption totals 2,085 active and under-age-65 retirees and 158 over-age-65 retirees, though vendors must guarantee quoted fees regardless of actual enrollment. MTA shall not pay network access fees for Medicare-eligible participants with Medicare as primary coverage. Offerors must provide a guaranteed provider aggregate discount percentage and specify the percentage of administration and network access fees at risk for failing to meet the discount guarantee, measured and assessed annually. All costs represent actual contractual amounts before copay or coinsurance consideration, with total projected expenses adjusted to account for assessments associated with provider discount guarantee shortfalls and ranking based on cumulative cost across all eleven contract years.
