STATEMENT OF WORK
Kyle Health Center Modernization Kyle, SD

1. Project Description
A. Title: Kyle Health Center Modernization
B. Location: All project elements shall be located at the Kyle Health Center in Kyle, South Dakota.
C. Period of Performance: 365 calendar days from Notice to Proceed.
D. Description: This Statement of Work outlines the replacement and upgrading of various architectural and infrastructure issues at the Kyle Health Center in Kyle, SD. During the FY22 annual general inspection and FY23 facility condition assessment, several deficiencies were documented, along with some infrastructure needs. The identified issues include:
a. Demolition and installation of wall system to include insulation, gypsum, tape, texture, and paint.
b. Demolition of the existing flooring system, which includes vinyl and tiles. This will be followed by the installation of hospital‐grade vinyl flooring throughout the facility, along with new vinyl cove base.
c. Facility‐wide painting and installation of wall protection measures to help reduce the number of Environment of Care findings and enhance aesthetics.
d. Demolition and replacement of the suspended ceiling grid system, tiles, and supply and return grilles.
e. Replacement of the original casework with medical‐grade casework and solid surface countertops.
f. Replacement of the atrium windows, exterior windows, and the storefront.
g. Replacement of various interior and exterior doors and frames.
h. Replacement of the front entrance sidewalk, ramps, and curb and gutter.
i. Installation of artificial turf, concrete sidewalks, outdoor benches, and a flagpole.
j. Demolition of various roofing systems and installation of standing seam metal.
k. Replacement of various bathroom fixtures and faucets, along with the partitions.
l. Refinishing the wooden glulam beams and ceiling planks.
m. Installation of a pilot‐activated lighting control box for the helipad lights.
n. Design and installation of facility‐wide signage.
None of these issues poses an immediate threat to our accreditation. The current condition of the flooring, paint chipping, and outdated casework has been noted.
E. Background: Most of the components are from the original construction and require replacement. The goal of this project is to modernize all components to comply with current healthcare standards, enhancing both patient care and satisfaction, as well as improving staff engagement in the upgraded facility.

2. Statement of Work:
The contractor shall provide all materials, labor, equipment, and supervision required to accomplish the following:
A. Warranty: The contractor shall provide a one‐year warranty from the time of substantial completion of the entirety of this project. The warranty shall include material, labor, travel, and expenses with no deductibles.
B. Demolition: The contractor shall assume responsibility for all materials and components designated for demolition during this project. All demolished materials must be disposed of in compliance with relevant regulations. The items to be demolished include various flooring systems, casework, windows, doors, roofing materials, bathroom fixtures, and concrete. For additional details, refer to the Construction Drawings.
C. Walls: The contractor shall install 5/8” type X gypsum for all walls, minus the two bathrooms. The insulation is unknown at this time, but the contractor shall install mineral wool blankets in the wall cavities. The walls shall be taped and orange‐peel textured prior to painting. Refer to plan sheet A103 for final wall configurations.
The half walls for the two nurse stations, RM313 and RM406, will have data and electrical outlets installed beneath the countertop. These utilities can be run through the crawlspace and emerge through the floor inside the half wall. The data must match the specifications of the existing infrastructure. Refer to plan A104, note 09‐07, for details.
D. Flooring and Covebase: The contractor shall demo, prepare, and install a new flooring system as noted in plan sheet A101. Additionally, the contractor shall install all products according to the manufacturer's specifications.
a. Flooring: Per note 09‐01, sheet A101, the contractor shall install Patcraft Pursue I633V vinyl flooring, color Restful‐V2 or equivalent. Approximately 13,000 square feet of flooring will be installed. The contractor must verify measurements before installation.
b. Covebase: Per note 09‐02, sheet A101, the contractor shall install Johnsonite 4‐inch vinyl base or equivalent, color Matte Black or equivalent. Approximately 6,200 linear feet of covebase will be installed. The contractor must verify measurements before installation.
E. Paint and Wall Protection: The contractor is required to repair walls to level 4 finish before painting and install wall protection as noted in plan sheet A101.

a. Paint: Per note 09‐03, sheet A101, the contractor shall paint all interior walls and ceiling, OC‐51 Intense White or equivalent. The contractor must verify before painting.
b. Wall Protection: Per note 09‐04, sheet A101, the contractor shall install InPro wall protection, color TBD or equivalent. Wall protection shall be installed after painting and before the covebase. In the corridors, the wall protection shall be a full sheet. In the areas that have casework, the wall protection shall be installed so that the top of the sheet is level with the backsplash. The contractor must verify measurements before installation.
F. Ceiling Grid: The contractor is responsible for demolishing and reinstalling the suspended grid system as noted in plan A102. The ceiling grid shall have a 2’x2’ layout suitable for 2’x2’ ceiling tiles. The grid system shall be Suprafine, color White or equivalent. The ceiling tiles shall be 9/16” beveled tegular, Ultima Health Zone High NRC, color White or equivalent. The height shall match the existing.
The contractor is responsible for replacing all the supply and return grilles, color White. Refer to the Mechanical plan sheet M‐5 for quantity and location.
Note: A contract is awarded for the replacement of most of the light fixtures. The light fixtures will measure 2’x4’, so the ceiling grid must be designed to accommodate these dimensions.
G. Casework: The contractor is responsible for demolishing the existing casework and installing new casework as noted in plans A103‐A118. The casework shall be Midmark with Storm cabinets and Arrowroot solid‐surface countertops or equivalent. The panel style shall be serenity, the handle type bent, and the handle finish brushed nickel. All the necessary details and dimensions have been provided on plans A104 through A118. The contractor is responsible for verifying the dimensions before ordering.
The sinks shall be stainless steel. The contractor is to verify dimensions before ordering materials.
H. Doors: The contractor is responsible for replacing the door listed on plans A103 and A121. The frames shall be painted Sherwin‐Williams Black Magic or equivalent. The door color shall be Select White Maple Alpine AL18 or equivalent to match the Pharmacy. The metal door shall be painted Sherwin‐ Williams Black Magic or equivalent. All interior doors shall be wrapped in 18‐gauge satin stainless‐steel, 34‐inch armor plates. All exterior doors shall have the 34‐inch armor plate installed on the interior of the door. The aluminum doors shall be Matte Black or equivalent. Refer to pages 26 and 27 of the Construction Drawings for more information. The contractor is to verify dimensions before ordering materials.
I. Atrium Windows: The contractor is responsible for the demolition and installation of new 1‐inch insulated glazing tempered windows for the atrium. The frames shall be matte black aluminum. A commercial film shall be applied, color TBD after award. For more information, refer to plan A119 and the Architectural plan sheets A‐7 and A‐15 for dimensions. The contractor is to verify dimensions before ordering materials.
J. Windows: The contractor is responsible for the demolition and installation of new exterior windows.
The frames shall be matte black aluminum. A commercial film for solar control and energy savings shall be applied, color TBD after award. The interior shall be gypsum returns with no trim. Refer to plan A121 for locations and the Architectural plan sheet A‐15 for dimensions. The contractor is to verify dimensions before ordering materials.
K. Storefront: The contractor is responsible for the demolition and installation of new storefront windows.
The frames shall be matte black aluminum. A commercial film for solar control and energy savings shall be applied, color TBD after award. Refer to plan A121 for locations and the Architectural plan sheet A‐ 15 for dimensions. The contractor is to verify dimensions before ordering materials.
L. Concrete: The contractor is responsible for the demolition and reconstruction of the sidewalk, ramp, curb, and gutter as detailed in plan A120. The ramp must comply with ABA standards and should include a thickened edge. The sidewalk must have a minimum thickness of 4 inches. Expansion and contraction

October 2025 joints are to be installed as necessary. The contractor is required to provide an as‐built built for the concrete, including specifications of the materials used. The contractor shall ensure that the drainage is away from the building foundation. The contractor is to install and anchor two outdoor courtyard benches, color TBD. The contractor is to install a new 40‐foot, 8‐inch, commercial flagpole with an internal rope halyard, color TBD. The contractor is to verify dimensions before ordering materials.
M. Artificial Turf: The contractor shall install artificial turf as shown on plan A120. The turf shall be Synlawn Synfescue 343 or equivalent. Positive drainage away from the building foundation. The contractor is to verify dimensions before ordering materials.
N. Misc. Roofs: The contractor is responsible for the demolition and installation of standing seam metal roofs on the administration building, approximately 2,000 sf., and the incinerator building, approximately 180 sf. The administration building also requires new gutters. The roof shall be Metal Sales Vertical Seam, color Regal Blue or equivalent to match the main entrance roof. The standing seam shall be 24‐gauge, 18‐inch panel width, and 1 ¾‐inch standing seam. The gutters shall be seamless to include downspouts, and the color White. The contractor shall replace the roof cap on the Incinerator building, color Blue to match the main entrance roof. The lightning protection system shall be upgraded as well. The contractor shall replace the tipi portion on the main entrance roof with seamless metal panels, color Blue. The lightning protection system on the tipi has been upgraded already and can be reinstalled. Refer to plan A122 for locations, the Architectural plan sheet A‐1 for dimensions of the incinerator roof, and page 19 of the Electrical plan sheets for the LPS. The contractor is to verify dimensions before ordering materials.
O. Bathrooms: The contractor is responsible for the demolition and installation of new bathroom fixtures, including faucets, toilets, urinals, grab bars, and partitions. The faucets, toilets, and urinals shall be high‐ efficiency, low‐flow, battery‐operated, and automatic. The partitions shall be stainless steel and hospital‐grade. ABA standards shall be incorporated where required. Refer to plan A123 for locations, the Mechanical plan sheet M‐2, and the Addition plan sheet PL101 for quantities and locations.
The casework, BA00 or equal, and the solid surface countertops shall match the specifications of Section G. The new sinks shall be stainless steel. The contractor is to verify dimensions before ordering materials.
All tile on the floor and walls shall be demolished and refinished to match the specifications outlined in Section C. Contractors are not required to install mineral wool insulation; however, they must ensure that the gypsum is moisture‐resistant.
P. Wood Beam and Ceiling: The contractor is responsible for refinishing the wooden glulam beam and ceiling planks in the main waiting area. The stain and finish are TBD after the award. The contractor shall also paint the fire sprinkler piping in this same area, color OC‐51 Intense White or equivalent. The sprinkler heads SHALL NOT be painted, only the piping. Refer to plan A124 for notes and the Architectural plan sheet A‐7 for wood plank locations, A‐22 for beam dimensions, and the Fire Sprinkler plan for pipe dimensions and locations. The contractor is to verify dimensions before ordering materials.
Q. Helipad Lighting: The contractor is responsible for installing and verifying a pilot‐activated lighting control box, which shall be located in a convenient location within the facility. Currently, the light switch that controls the helipad lighting is located in room 309. The contractor is to verify electrical requirements and refer to the electrical schedules for available power. The contractor shall install the control box in the vicinity of the nurse station, room 326.
R. Signage: The contractor is responsible for designing and installing facility‐wide signage. An example of recently designed signage from a neighboring hospital can be provided for guidance and inspiration. For example, refer to the Signage Example.

Note: The signage example is only a sample. Department names, color, design, translations, and quantity may vary. Exterior signage is to be included in the design.
S. Clean Up: The job site must be cleaned daily and adhere to the requirements set by the Infection Control & Safety Officer as outlined in the ICRA and PRCA. All construction areas should be free of debris, rubbish, and leftover material scraps upon project completion. The contractor is responsible for providing trash receptacles for collecting this debris and for ensuring its removal from the project site.
The COR will inspect the site before final payment to confirm that it has returned to its original condition.
T. Site Restoration: The construction site must be clean of all materials, debris, and rubbish. It is the contractor's responsibility to remove any such debris and restore any property outside the construction limits, as approved by the COR, to its original condition. Before the start of construction, the contractor shall take photographs to document the site's original condition and provide the photographs to the project COR prior to any on‐site work.
U. Additional Facility Requirements: The contractor shall comply with the requirements of the Facility Manager, Infection Control Officer, and Safety Officer. These requirements will be laid out during the pre‐construction meeting. Other permits and requirements will be given after the pre‐construction meeting, when the ICRA and PRCA assessments are completed. An ICRA assessment was performed for this project, and a map of the ICRA levels throughout the facility for the scope of work presented in this project is provided.

4. Schedule/Meetings
A. The contractor shall submit a detailed schedule, in Gantt chart form, to the COR for approval within 14 calendar days of the Notice to Proceed.
B. Progress Meetings: Progress meetings shall be conducted at regular intervals as merited by the progress of the work, but no less than every other week.
C. Substantial Completion: Following the general contractor's notification of the COR that the project is substantially complete, one substantial completion meeting shall be held. Substantial completion means the project is fit for its intended use, with only minor, lingering details to finish. The owner can use the facility when a project is substantially complete.
D. Final Completion: One completion meeting shall be held following the determination that the project is 100% complete. After the final inspection, if there are remaining punch list items to be completed, the Contracting Officer will determine the number of days allowable for the Contractor to complete the punch list.
5. Miscellaneous
A. Security Requirements:
a. All contractor employees shall check in with the Facility Manager/House Manager before commencing work in the Kyle Health Center.
b. The contractor shall be fully responsible for the security of the work site. The hospital and the Indian Health Service staff will not be responsible for securing the contractor’s tools, equipment, or materials during the project.
B. Occupancy of Government Facilities. The contractor shall be responsible for properly informing the COR of any condition that would jeopardize the building or its contents, such as heat loss, air conditioning, danger of freezing pipes, electrical power, water, sewer, or building security.
C. Temporary Utilities: The contractor shall be responsible for providing all temporary utilities related to this project.
D. Tribal Requirements: OGLALA SIOUX TRIBE
a. Contractors are notified that this project will be performed on the Pine Ridge Indian Reservation, home to the Oglala Sioux Tribe. Contractors are required to contact the Tribal Employment Rights Office (TERO) upon receipt of notification of award. Compliance with the tribal employment ordinance is required prior to starting work on any project performed on Tribal Land, including payment of associated TERO fees/permits. Utilization of Tribal individuals will be a requirement of hiring goals for a set craft or skill level.
b. The Contractor is responsible for paying all applicable tribe fees and/or taxes, or applicable requirements that may be incurred as a result of this project and must be included in the firm fixed price offered on the SF1442. Failure to include applicable tribe fees and/or taxes in the firm fixed price shall not constitute grounds for an equitable adjustment after award.
c. Contractor shall obtain a business license, permits (if required by the tribe), and pay the required fees to the appropriate office of the Oglala Sioux Tribe. The general contractor and each subcontractor are required to obtain a business license on each project prior to starting work.

E. Construction Materials and Storage
a. All materials shall be stored and handled according to the manufacturer’s recommendations.
b. The contractor(s) shall be present and on‐site to receive all construction materials, equipment, or products. IHS staff will not be present and responsible, nor will they obtain or provide offloading equipment for construction materials, products, or equipment.
c. The contractor(s) shall provide the means of storing any construction materials, products, or equipment required for installation. IHS staff will not be responsible for or provide any means of storing construction material, products, or equipment before or during construction.
