L.3 INSTRUCTIONS FOR PREPARATION AND SUBMISSION OF PROPOSALS
(a) Each Offeror will submit information identified in the sections as set forth in paragraph (c) below. The data submitted should be complete, concise and relevant to the requirements of the SIR/RFO and are required to be submitted in the prescribed formats subsequently identified herein.
(b) Documentation shall be submitted electronically in accordance with Section L, Provision 3.2.2.3-20 Electronic Offers. Note: Section I documentation shall be submitted as a separate document from Section II documentation.
(c) The titles and contents of the sections should be as set forth below.
Proposal Organization Section Title I Contract Documentation/Price Proposal II Past Performance Information* *No reference shall be made to prices in Section II.
(d) Section cover shall designate section title along with applicable page(s) the Offeror deems competition sensitive.
(e) Section I - Contract Documentation: This section will be in FAA format. This will provide information to the FAA for preparing the contract document and supporting file.
Offerors must complete and submit:
(1) Section A, Solicitation, Offer and Award (SF33), Blocks 12 through 18;
(2) Section B, Supplies or Services and Prices/Costs; Government format (Reference SIR Attachment 1)
(3) Section K, Representations and Certifications; SIR Attachment 12, OCAT 00320 BUSINESS RELATIONSHIP AFFIDAVIT; SIR Attachment 13, OCAT 00310 ANTI/NON-COLLUSION AFFIDAVIT, and, (Note: Completion of documents identified at (1) through (3) above indicates that the Offeror has read and agrees to the terms and conditions contained in the SIR/RFO.
The FAA may consider offers that take exception to the terms and conditions of the SIR/RFO to be unacceptable and thereby ineligible for award. Offerors submitting such offers may not be given the opportunity to revise their offer.)
(4) Proposal Guarantee
(A) A copy of the Proposal Guarantee will be provided electronically with the proposal.
In accordance with Section I, Clause 3.4.1-1 Proposal Guarantee, an original, signed Proposal Guarantee shall be submitted by the due date and time for receipt of proposals to the following address:

(5) Small Business and Small Disadvantaged Business Subcontracting Plan (required for offerors who are a large business)
(f) Section II – Past Performance Information The Offeror (General Contractor), as well as proposed Mechanical and Electrical sub-contractors, are required to provide Past Performance Surveys (PPS) (Section J, Attachment 11), along with Client Authorization Letters (format at Section J, Attachment 12) to past customers/clients. The Offeror, along with their Mechanical and Electrical subcontractors, will send Past Performance Surveys (PPS) to clients who are familiar with the work demonstrated through previous contracts of a similar nature.
Only renovation projects completed within the last five (5) years will be considered;
however, one (1) project that is currently in progress and at least 50% complete maybe considered for no more than one (1) PPS requirement for the general contractor as well as for the electrical subcontractor and the mechanical subcontractor. The PPS will be returned directly to the Contracting Officer. Offerors shall request a minimum of two
(2) with a maximum of four (4) Past Performance Surveys as a general contractor as well as a minimum of two (2) with a maximum of four (4) Past Performance Surveys for their mechanical subcontractor and a minimum of two (2) with a maximum of four (4) Past Performance Surveys for their electrical subcontractor.
The Offeror (General Contractor) shall provide, at a minimum, the following information for each PPS primary reference for each project. The Offeror shall also provide this information for an alternate reference related for each project. Note: Alteration and or deletion of data fields contained in the PPS may result in the offeror being eliminated from further consideration.

• Evidence to show that the PPS was provided to the primary reference and alternate reference by no later than 10 days prior to the SIR closing date along with instructions to provide the completed PPS to the Contracting Officer by no later than the SIR closing date. This evidence shall be provided for all PPS for the General Contractor as well as Mechanical subcontractors and Electrical subcontractors.
The Offeror and the proposed Mechanical and Electrical subcontractors will complete Page 1 of the PPS and then provide the surveys to their references, who will in turn complete and return the surveys directly to the Contracting Officer by the SIR closing date.
CAUTION: Offerors are cautioned that the FAA may use the data provided by other sources in evaluating past performance. Offerors will be given an opportunity to rebut information considered negative and relevant to the evaluation. While the FAA may consider data from other sources, the burden of providing thorough and complete past performance data rests with the Offeror.
The PPS will be evaluated utilizing the five level assessment rating of “Exceptional”, “Very Good”, “Satisfactory”, “Marginal” or “Unsatisfactory” defined as follows:
Rating Definition
Exceptional
Performance meets contractual requirements and exceeds many to the Government’s/Owner’s benefit. The category being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Very Good
Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit. The category being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Satisfactory
Performance meets contractual requirements. The category being assessed contains some minor problems for which corrective actions taken by the contractor appear to be or were satisfactory.
Marginal
Performance does not meet some contractual requirements. The category being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The category being assessed contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

PRE-PROPOSAL CONFERENCE QUESTIONS AND ANSWERS
JUNE 29, 2020
Question 1: Referencing Attachment 10 – BMB Past Performance Survey, the cover sheet requests that recipients provide a response no later than April 10, 2020 at 2 PM CST, which has elapsed. Please clarify the precise date/time by which respondents need to return their Performance Survey to the Contracting Officer.
Answer: The Revised BMB Past Performance Survey (PPS), dated June 26, 2020, included in SIR Amendment 0001 revises the response date for receipt of completed PPS to July 20, 2020.
Question 2: Please advise what the Project Solicitation 'Construction Cost Limit' or 'Cost Ceiling' or 'Anticipated Funding Request Limit' for the Project is. We are concerned that the base proposal value will exceed the 'Magnitude of Construction:
$5,000,000.00 to $10,000,000.00 after review of the Contract Documents."
Answer 2: The FAA projects that the construction cost for this acquisition will be within the stated Magnitude of Construction of $5,000,000.00 to $10,000,000.00.
Question 3: Referencing Section L, paragraph L.3, the requirement for number 3 reads as follows:
“(3) Section K, Representations and Certifications; SIR Attachment 12, OCAT 00320 BUSINESS RELATIONSHIP AFFIDAVIT; SIR Attachment 13, OCAT 00310 ANTI/NON-COLLUSION AFFIDAVIT, and,”
Please advise if this is a complete list of what is required to satisfy section 3, or whether additional information is required for Item 3.
Answer 3: Provision L.3, subparagraph (3) is being revised to add the requirement for a Subcontracting Plan for those offerors that are large businesses.
Question 4: Referencing Section L, paragraph L.3, page 68, a list of minimum criteria is provided for submission in our proposal. This is very similar but not identical to that information requested on page 2 by the offeror on the Past Performance Survey. Is the intent for the offeror to complete page 2 and provide in the packet that will ultimately be completed by the client and sent to the contracting officer, and then address the list separately in our proposal, or to provide both the list and page two of the past performance survey in our proposal?
Answer 4: Offerors shall complete page 2 of the PPS and provide the PPS to the primary reference and alternate reference. Offerors shall also provide the information contained in Provision L.3 (listed on Page 68 of the SIR). Primary and alternate references will be providing the completed PPS package, including the information listed on page 2 of the PPS, when the completed PPS is provided to the Contracting Officer.
Question 5: Is the intent to send the PPS Survey concurrently to both the primary and alternate references?
Answer 5: Correct
Question 6: Referencing Section L, paragraph L.3, page 70, it is indicated to provide a Small Business & Small Disadvantaged Contracting Plan. However, this is not identified in the list of submittal criterial for L.3. Please identify whether the offeror is to submit this with our proposal, and, if so, which section and factor.
Answer 6: Provision L.3, subparagraph (3) is being revised to add the requirement for a Subcontracting Plan for those offerors that are large businesses.
Question 7: Referencing Section L, paragraph L.3, references are made to Volume I documentation and Volume II documentation; however, volumes are not identified.
Please confirm that the sections that same thing as the volumes (i.e., Section II – Past Performance and Section – Contract Documentation).
Answer 7: References to Volume I and Volume II in Provision L.3 will be corrected to Section I and Section II.
Question 8: We understand that offerors shall provide evidence that the offeror’s PPS submission was provided by no later than 10 days prior to the SIR closing date, along with instructions to provide the completed PPS to the contracting office no later than the SIR closing date. Please clarify whether this is business days or calendar days.
Answer: Calendar days
Question 9: Please identify what satisfies the requirement to provide the evidence that the offeror’s PPS submission was provided by no later than 10 days prior to the SIR closing date, along with instructions to provide the completed PPS to the contracting office no later than the SIR closing date. Does the KO prefer a copy of the outgoing email, or will Page 1 of the Past Performance Survey suffice?
Answer 9: The contracting Officer will accept a copy of the outgoing email provided that is shows the time and date that the email was sent.
