About this file

This document is an instruction guide issued by the Commonwealth of Pennsylvania Department of Environmental Protection (DEP) Bureau of Air Quality for completing asbestos abatement and demolition/renovation notifications. The form can satisfy notification requirements for multiple agencies including the PA DEP, Allegheny County Health Department, PA Department of Labor and Industry, and the City of Philadelphia Department of Public Health. The notification process applies to projects subject to 40 CFR Part 61, Subpart M (National Emission Standards for Hazardous Air Pollutants) or the Asbestos Occupations Accreditation and Certification Act (Act 194). All applicable sections of the notification must be completed with original signatures, as faxed copies are not accepted. Revisions to previous notifications must be highlighted. The instructions specify requirements for Items 5, 6, 12, 13, 17, 18, 26, and 27, and note that additional space for descriptive text may be continued on blank sheets as attachments.

Initial notifications must be accompanied by applicable fees: $300 for projects in Pennsylvania counties except Philadelphia or Allegheny (2021-2025), $400 (2026-2030), and $500 (2031), payable to "The Commonwealth of Pennsylvania Clean Air Fund." For Allegheny County and Philadelphia projects, separate fee schedules apply. No fee is required for notification revisions or fire set projects for firefighting instruction purposes. Notifications must be submitted to the appropriate regional DEP office based on facility location, with specific addresses provided for Southeast, Southcentral, Southwest, Northeast, Northcentral, and Northwest regions. Allegheny County requires a permit if the project involves at least 260 linear feet or 160 square feet of asbestos-containing material; Philadelphia requires a permit for 80 or more square feet or 40 or more linear feet of friable asbestos-containing material. Philadelphia submissions require three copies; if a permit application is required, it must be approved prior to project start and two copies must be included with the notification. Facility inspection surveys must be included for all demolition projects.
