About this file
This is a Forced Labor of Ethnic Uyghurs Ban certification document issued by the State of Arizona for Solicitation No. BPM005719, related to bulk thermoplastic and thermoplastic preformed pavement markings procurement. The document outlines Arizona's compliance requirements under A.R.S. 35-394, which prohibits public entities from contracting with companies that use forced labor of ethnic Uyghurs in the People's Republic of China or goods and services produced by such labor. The certification applies to organizations with ten or more full-time employees and requires all offerors to select one of three compliance options: certify non-use of forced Uyghur labor and related goods or services, declare participation in such labor, or claim exemption based on business structure. Crown USA Inc., located at 35 Industrial Park Drive, Woodbury, Georgia, submitted this certification dated October 26, 2023, signed by Donnie Kick, President.
The document establishes exemption criteria for sole proprietorships, contractors with fewer than ten employees, and non-profit organizations, which are not subject to the forced labor certification requirement. Offerors must make a binding attestation regarding their compliance status or declare their exemption eligibility at the time of bid submission. This certification requirement applies throughout the contract term and extends to all contractors, subcontractors, and suppliers involved in the performance of work, ensuring comprehensive supply chain compliance with Arizona's forced labor prohibition statute.
