About this file

This document is Attachment A: Authorization & Certification for a Request for Proposals (RFP) issued by the City of Long Beach. The attachment establishes the authorization requirements and federal compliance certifications necessary for proposers to submit bids for goods and services contracts with the city.

The authorization section specifies signature requirements based on business entity type: sole proprietorships require only the owner's signature; partnerships require signatures from general partners; corporations require two officer signatures or one officer signature accompanied by a certified corporate resolution; and limited liability companies require a member or manager signature. Proposers must certify that they have read and agree to comply with all RFP terms and conditions, abide by the City of Long Beach Ethics Handbook, and submit genuine proposals valid for 90 days from the submission deadline. Additionally, proposers must certify they are not engaged in conflicts of interest and have not engaged in collusive bidding practices.

The document includes federal debarment and suspension certifications required under Executive Order No. 12549 and Title 2 CFR Part 180 Subpart C, mandating that proposers and their principals certify they are not currently suspended, debarred, or ineligible for federal funds; have not been suspended or debarred within the preceding three years; do not have pending debarment proceedings; have not been indicted or convicted of fraud or misconduct within three years; and have not had public transactions terminated for cause or default within three years. The document also includes a Local Preference Provision allowing Long Beach-based businesses meeting specified criteria to receive a five percentage point increase to their evaluation score, contingent upon submission of a current valid business license and seller's permit from the City of Long Beach.
