About this opportunity
The Department of State's Bureau of Global Health Security and Diplomacy is soliciting applications through an Annual Program Statement to advance the America First Global Health Strategy by funding projects that strengthen health systems, enhance efficiency, and foster country self-reliance across multiple partner countries, with the next Statement of Interest deadline on February 14, 2027.
Eligible applicants include U.S. and foreign private for-profit enterprises, institutions of higher education, NGOs including faith-based organizations, public international organizations, and governmental institutions. Cost sharing is not required but may be proposed and discussed during negotiations. The program operates on a two-step process beginning with a Statement of Interest submission (maximum five pages or 2,500 words) that is evaluated on program idea quality, organizational capacity and previous grant experience, and ability to achieve objectives. Selected SOIs may advance to consultative program design with GHSD or proceed directly to Phase 2 for full proposal submission.
Award amounts range from $500,000 to $250 million, with up to 100 awards anticipated from total estimated funding of $4.5 billion subject to availability. Performance periods extend up to five years. Country-specific addenda signal distinct priorities and funding allocations: Nigeria (up to 20 awards, $200 million total); Uganda (up to 15 awards, $60 million); Mozambique (up to 15 awards, $180 million); Malawi (up to 4 awards, $35 million); Côte d'Ivoire (up to 3 awards, $50 million); Cameroon (up to 3 awards, $5.4 million); and Innovations in Population-Based Surveys and Surveillance (up to 4 awards, $80 million). Objectives vary by addendum but emphasize integrated service delivery across HIV, tuberculosis, malaria, maternal and newborn child health, surveillance systems strengthening, digital health interoperability, supply chain capacity, and health workforce sustainability with transition to government-led financing. All projects must align with bilateral Memoranda of Understanding and the America First Global Health Strategy, prioritizing country ownership, local partnerships, continuity of life-saving services, and evidence-based approaches. Applicants must obtain a Unique Entity Identifier and register in SAM.gov before submitting Phase 2 applications. Submissions are accepted via Grants.gov or MyGrants on a rolling basis with specific quarterly windows; applicants should not submit identical SOIs on both platforms.

Synopsis
The Department of State invites eligible applicants to advance the America First Global Health Strategy, which aims to save lives, strengthen health systems, enhance efficiency, foster self-reliance, and ensure U.S. investments benefit American safety, strength, and prosperity. This Annual Program Statement (APS) establishes a supplemental framework through which the Department of State may identify and support projects that complement, extend, and/or fill identified gaps in the implementation of these bilateral MOUs. Through specific Addenda, the Department will signal priorities and needs. This APS provides the standard application instructions for the submission of all Statements of Interest (SOIs) to these specific Addenda. GHSD will collaborate with Embassies and other Department of State Bureaus and Offices to post specific funding opportunities through Addenda to this APS that address health challenges and priorities of importance.
