About this opportunity

The Department of State Bureau of Global Health Security and Diplomacy is soliciting applications through an Annual Program Statement (APS) to advance the America First Global Health Strategy across multiple country-specific initiatives, with rolling submission windows and a principal deadline of February 14, 2027 for Statements of Interest. Applicants are unrestricted by organizational type; eligible categories include U.S. and foreign for-profit enterprises, institutions of higher education, non-profit organizations including faith-based entities, public international organizations, and governmental institutions.

The program operates as a two-phase process requiring initial submission of a five-page maximum Statement of Interest (SOI) describing project approach and outcomes, with selected applicants advancing to full proposal development. SOI submission windows occur quarterly, with specific country addenda issued throughout the year. The most active current addenda carry these parameters: Cameroon (Addendum G, due September 15, 2026) offers up to $5.4 million across up to three awards for strengthening integrated disease surveillance and emergency response; Cote d'Ivoire (Addendum H, due September 15, 2026) provides up to $50 million across up to three awards for surveillance, service delivery integration, and health systems transition; Mozambique (Addendum J, due September 15, 2026) allocates up to $180 million across up to 15 awards for integrated service delivery and health security; Population Surveys and Surveillance (Addendum I, due September 15, 2026) offers up to $80 million across up to four awards for innovative methodologies; Neglected Tropical Diseases (Addendum K, due October 30, 2026) provides up to $162 million across up to 25 awards targeting 31 countries; and Nutrition (Addendum L, due October 30, 2026) allocates approximately $115 million across up to 12 awards in 12 sub-Saharan African and Caribbean countries. All projects must be completed within five years or less. Individual awards range from $500,000 to $250 million, with total program funding of up to $4.5 billion across anticipated 100 awards, subject to availability.

Cost-sharing is optional and will not be evaluated in merit review. Organizations must obtain a Unique Entity Identifier and maintain active SAM.gov registration before Phase 2 submission but not for Phase 1 SOI filing. Merit review of SOIs evaluates program idea quality, organizational capacity and grants history, and program planning ability, weighted equally. Awards will be issued as grants, cooperative agreements, or fixed-amount awards. Recipients must comply with 2 CFR Part 200, trafficking in persons certifications for awards exceeding $500,000 in services outside the U.S., and Leahy Law vetting for security force assistance. All country addenda emphasize country ownership, sustainability planning, local engagement, evidence-based approaches, and integration with existing government systems and prior U.S. investments. The opportunity is assigned to Assistance Listing 19.029 (U.S. President's Emergency Plan for AIDS Relief Programs).

Synopsis

The Department of State invites eligible applicants to advance the America First Global Health Strategy, which aims to save lives, strengthen health systems, enhance efficiency, foster self-reliance, and ensure U.S. investments benefit American safety, strength, and prosperity. This Annual Program Statement (APS) establishes a supplemental framework through which the Department of State may identify and support projects that complement, extend, and/or fill identified gaps in the implementation of these bilateral MOUs. Through specific Addenda, the Department will signal priorities and needs. This APS provides the standard application instructions for the submission of all Statements of Interest (SOIs) to these specific Addenda. GHSD will collaborate with Embassies and other Department of State Bureaus and Offices to post specific funding opportunities through Addenda to this APS that address health challenges and priorities of importance.
