About this file

This file is a compilation of daily field reports and weekly summaries from October 2015 through January 2016 documenting drilling, grouting, and foundation work activities at the Rough River Dam Safety Modification Project in Falls of Rough, KY. The reports detail extensive drilling operations using sonic rigs, core drills, and Cubex rigs to install sleeveport pipes and grout various holes as part of the dam's foundation work. Specific activities included drilling through embankment soils to reach bedrock, installing PVC sleeveport pipes with grout bags and fabric, conducting staged grouting operations with different grout mixes (A through E), performing televiewer surveys, and addressing various technical challenges like stuck drill rods, grout leakage, and gas pockets.

The reports contain detailed documentation of daily hole depths, grout quantities used, pressure readings, drilling issues encountered, and quality control testing of grout mixes. Multiple drilling crews worked simultaneously using different rigs across various hole locations designated by alphanumeric codes (e.g., DXP2502, UXP1816). The work required careful coordination between drilling contractors (Cascade) and grouting contractors (ACT) to properly sequence drilling, PVC installation, and multi-stage grouting operations. Technical oversight was provided by Corps of Engineers personnel who monitored operations and made key decisions regarding drilling depths, grout mix changes, and acceptance of completed work.

Rough River Phase 1B – Weekly Summary 10/26/15 – 11/1/15

Completed Work

This should generally be a list of what has been completed every day for the previous week by date. No details, just notes. Generally this should be completed on a daily basis by the field geologist at the end of the each day. The next morning ED should be able to review to determine what was done and what data needs reviewed that day.

Monday, 10/26/15:

Continue Grout Mix testing. A and B mixes were completed on Friday. C and D mixes were tested today.

Grout cubes were made by QA’s for all mixes. Michael Xu completed all grout testing. During grout mix testing, IntelliSystem performed checks for head loss for the equations used for calculating target pressures for grout/water pressure tests. The checks were performed after each ideal mix was completed, using a grout cart and circulating grout through their lines.

Tuesday, 10/27/15:

Continue Grout Mix testing. The sanded mix (E-mix) was tested today. ACT made two batches of D mix and then sent the mix to the portable grout cart. Michael Xu tested the grout. The sanded mix needed 6 buckets of sand to have a flow cone value of 12 seconds. Cylinders were made by QA’s. IntelliSystem performed their checks.

Wednesday, 10/28/15:

Henning pumped out the pond in the disposal area. ACT worked on the tent cover for the north grout plant (making the plant fully enclosed). ACT began building the walls of the south grout plant. The batch plant for the south grout plant arrived this afternoon. A two man crew, using a forklift, reinstalled the posts for the guard rail that had been removed by Cascade.

Thursday. 10/29/15: No Work

Friday, 10/30/15: No Work

Saturday, 10/31/15: No Work

Sunday, 11/1/15: No Work

Weekly Summary The PDT will meet on Monday afternoons to discuss the previous weeks work and if there any issues that need to be discussed, analyzed, or further studied.

- If there are major issues noted on the individual hole sheets from the previous week they should be discussed at the PDT meeting and at least noted on this sheet.

- This sheet serves as a chronological summary of work, whereas the hole sheet serves as a special summary of work.

- Notes may be made on this sheet by individuals throughout the week as a reminder for discussion at the PDT meeting.

- A summary of the PDT discussion should be written by ED (Geologist or Geotech depending on major items of work) after the PDT meeting

11/2/15 - 11/8/15 Completed Work

Monday, 11/2/15:

No work on the dam itself. Some grout mix testing. Cascade mobilized to site and participated in training prior to setting up the drill rig.

Tuesday, 11/3/15:

ACT performed a test for the grout bag (used at top of rock). They put a sleeveport pipe with a grout bag into a larger diameter PVC that had been cut in two, lengthwise). The larger diameter PVC was then sealed shut (making the larger diameter PVC the equivalent of the hole. ACT mixed two batches of A-modified grout (grout with bleed), and then grouted the grout bag. The larger diameter pipe was then taken apart after a 15 minute wait.

Cascade had jobsite safety training in the morning, followed by checks to the drill rig. Cascade drove the drill rig and pipe truck onto the platform at 1500, and began setting up on hole DP2502

Wednesday, 11/4/15:

Cascade began drilling DP2502 at 0739. At 13 feet there was a very weathered rock and clay mix soil.

During the next run, the driller encountered competent rock at 17 feet, shortly after a drill rod broke.

When the driller was about to stop (at 19 feet), another drill rod broke. Not all of the sample was retrieved, so they went back in the hole and drilled a little more to retrieve the sample. A third drill rod was broken. The drilled depth is 20.5 feet. The tape depth is 19.5 feet. Drilling complete at 0849.

Cascade then had to wait on ACT.

ACT did another grout bag test. This time the outer black PVC was secured very well and ACT was able to pressure up to 35 psi and hold for 20 minutes.

Cascade put the sleeveport pipe in the hole at 1011. ACT batched more A-modified-mix. At 1030 the double packer was put into the hole and grouting began, including the 20 minute hold time. The grout bag was complete at 1112. ACT washed out the lines and went to lunch.

After lunch, ACT began mixing B-mix grout. Began annular space grouting at 1340. Cascade removed the outer casing. ACT topped off the hole. At 1357 the hole was completely grouted with 25 gal. Cascade moved off the hole and onto new hole DP2462.

Cascade began drilling DP2462 at approximately 1420. At 1455 they were drilling in rock and at 1506 the hole was complete. The top of rock is at 28 feet, and the bottom of the hole is at 32 feet. A thin vertical fracture (about 6 inches in length) was at approximately 30.5 to 31 feet. The crew inserted the sleeveport pipe and ended shift for the day.

ACT tested a second A-mix (different than the original A-mix). The viscosity was 37 seconds, density was

1.41 g/cm^3, and the temperature of the grout was 75.6 degrees. The QC also made cylinders, tested bleed, tested the pressure filtration as well as the initial and final set times and the shear strength.

Thursday. 11/5/15:

(Per SS) Cascade was set up on hole DXP2462 from the previous day. From 0700-0830 the small mixer for the “modified A mix” was having a mechanical issue. The crew dumped the wasted grout bins while the issue was resolved. Top of rock was encountered at 28 fbgs, and the hole was socketed 4 feet into the bedrock at a depth of 32fbgs. The sleeveport pipe was set on the previous day with the sleeveport (annulus packer) installed at 27 fbgs. At 0910 the double packer (whip) was put into the hole and grouting began. There was an issue with leakage so the double packer was deflated and reset.

Eventually the 20 minute hold time at 35 psi was achieved at 0950. The hoses were cleaned. The drillers mistakenly removed 1 – 10 foot stick of casing thinking annulus grouting was complete. QC staff corrected the drillers and the bottom 10 feet of the hole (CH soils) remained open. B-mix was batched and placed for annulus grouting starting at 1015. The tip was set 6 feet from the bottom. At 1020 end grouting, pull remaining casing, and top off the hole. The total quantity of grout for annulus grouting was 76 gal. No other anomalies were identified.

At 1100 Cascade set up on DXP2422. There were no surprises during drilling of this hole as soils were predominately fill clay over bedrock. Top of rock was encountered at 38 fbgs, and the hole was socketed 4 feet into the bedrock at a taped depth of 42 fbgs. The draft log mistakenly recorded this as 41 fbgs (drilled depth) but will be corrected on the final log. The sleeveport pipe was installed at 37 fbgs. At 1300 Shifflett and Dorsey went to lunch and Adam Scheiter took watch. I was not present for grouting of the sleeveport pipe annulus bag. Dorsey and I arrived back on site for the completion of the annulus grouting at 1445. The total quantity of grout for annulus grouting was 65 gal. The rig was relocated to DXP2482 and the annulus space was topped off with B Mix.

Cascade began setting up on DXP2482 at 1430. Drilling commenced around 1450. Soils encountered included crushed stone fill overlying a brief zone of topsoil, overlying embankment clay over sandstone.

Top of rock was encountered at 18 fbgs with the hole socketed 3 feet into sandstone. The sleeveport pipe annulus bag was installed at 17 fbgs. The hole will be cleaned and taped in the am. Drilling complete at 1630. The sleeveport pipe was not installed.

Friday, 11/6/15:

Per Steven Shifflett - 0730 Contractor cleaned hole DXP2482 and inserted 30 feet of the PVC sleevepipe with the bag located at 17 fbgs. Batch plant began mixing modified A mix for the annulus bag at 0730.

0755 first batch was prepared. No QC Tests were observed. 0810 set the whip into the bottom of the hole with the tip set 1 foot above the bottom. At 0813 the Contractor began inflating the annulus bag on the sleeveport with a flow rate ranging from 1 – 2 gpm. The pressure was held for 17 minutes until 0830.

Adam Scheiter addressed the issue and the shorter time was the result of confusion from the work crew about what time the hold pressure was obtained. The Contractor flushed all lines and began batching the B mix.

QA/QC Tests of B mix at 0940.

Marsh Funnel B Mix test 1: QC=55s QA=59s. Marsh Funnel B Mix test 2: QC=56s QA=65s. Differences between QA and QC test results were discussed and examined. Marsh funnels were both cleaned and water tested to produce 26 second flow. Add 14L of water to the mix.

Marsh Funnel B Mix test 3: QC=48s QA=52s. Mud Balance B mix: QC=1.46 g/cm^3, QA=1.46 g/cm^3 Annulus grouting begin at 1010 with the whip set 5’ above the bottom of the hole. Hole complete at 1017 with an effective injection pressure of 7-11 psi and a flow rate of 5-6 gpm. Per Adam Scheiter the annulus grouting took 108 gal of grout. The casing was pulled and the hole was finished topping off at 1038.

At 1030 Cascade set up on hole DXP2442. At 1052 casing advancement began. Soils consisted of crushed limestone fill over clay embankment. At approximately 27-30 fbgs an organic zone was encountered.

Soils above the top of rock included zones of cobbles and a few boulders at30-33 fbgs. Top of rock was encountered at 36 fbgs and the socket depth extended to 39 fbgs into competent bedrock.

The sleeveport pipe was inserted with the annulus bag set at a depth of 35 fbgs. Modified Grout Mix A for the bag inflation was mixed and the bag was pressurized at 35 psi from 1225-1245. Full return on the annulus bag occurred at 1245. The lines were flushed and the batch plant began preparing grout mix B.

At 1212 QC testing of grout mix B occurred. The grout had a temperature of 73.1 degrees F, potable water temperature of 65 degrees F, mud balance of 1.50 g/cm^3, and a marsh funnel time of 46 S. At 1323 ACT inserted the whip for annulus grouting at the bottom of the first packer at 35 fbgs. Grouting occurred with effective pressures ranging from 14-22psi and flow rates between 5-6 gpm. Per Adam Scheiter the hole took 103 gal of grout. At 1335 Cascade began pulling casing to 1400 when the hole was topped off.

At 1430 Cascade set up on hole DXP2402 and began installing casing. There was a small zone of crushed limestone fill over typical embankment clays. An organic zone was noted to occur approximately 30-33 fbgs. At 1500 top of rock was encountered at a depth of 48 fbgs. The socket into the bedrock extended to 52 fbgs.

Saturday, 11/7/15:

Per Steven Shifflett- Hole DXP2402 hole summary: TOR 48 fbgs and 52 fbgs bottom of hole. Annulus bag was set to 47 fbgs and they cut 2 feet off top and put on bottom of sleeve port. At 805 ACT hit 35 psi and held the pressure until 825. ACT flushed the lines and switched to B mix. The sleeveport pipe was inserted at 0922 and grouting of B mix began. Cascade pulled the casing at 0938 when grout return at the top of the casing was observed. ACT topped off the hole beginning at 0950. B mix annulus grouting was complete at 1005. QC test at 0910 marsh funnel 47s. Potable Water Temperature 66.2 degrees F. Mud balance 1.50 g/cm^3.

At 1025 Cascade began drilling DXP2302. At 1152 they were at 55 feet with soils consisting of mainly clay with sandy clay zones. An organic zone was observed between 70 and 75fbgs but the transitioned back to embankment clays to 88 fbgs. Hit weathered rock from 88 to 103 fbgs at 1245. The socket depth was obtained at a depth of 105 fbgs at 1500. The annulus bag was set at 89 fbgs because the outer casing was set to 90 fbgs because it refused on weathered boulders and cobbles. 1515 begin setting sleeveport. 1530 complete. Double packer installed at 1535. Begin grouting at 1540. Stop grouting 1542. Full return while hold pressure is discussed. Whip set with tip 11 feet off bottom. 1600 to 1620 grout annulus bag with A modified grout and hold to 35 psi. Pull whip out at 1625. The hole depth required that the grout annulus of the sleep port pipe be grouted in 2 stages. 1st stage initiated at 1645 with a theoretical depth of 50 fbgs. 1700 Contractor accidently wasted 1 batch of grout by pushing the wrong button and adding too much water. 1 batch wasted by contractor, cost 30 to 45 minutes of production. SSHO required artificial lighting at 1630.

QC test annulus grout new A mix on subsequent re-batch in the 2nd tank- marsh funnel : 38.5 s, raw water temp 76.4 degrees F, Mud balance SG 1.43 g/cm^3, Potable Water temp 64 degrees F. Set whip at a depth of 15 feet above top of rock with a stage elevation target depth of 45 fbgs. Begin pump stage 1 annulus grout at 1715. Terminate port not aligned properly. Reset whip. Begin grouting at 1720. Full return at 1737. Pull casing leaving 50 feet in ground for 2nd stage.

Sunday, 11/8/15:

Per Steven Shifflett – 0743 ACT starts 2nd stage grouting on hole DXP2302. ACT missed the port location and went to full return at 0746. The whip was raised 10 additional feet. ACT discussed on the radios that in the future multi stage holes will have additional ports in the sleevepipe to alleviate potential blockages. ACT then had to pull the whip up an additional 3 feet to 32 feet below the top of the casing. ACT then had to reset whip to 35’ below casing measured to the bottom of the first packer because grout would not flow. This lined up with the port hole and grouting commenced at 0811 but ACT had to go to full return at 0818 to mix additional grout. Grouting of the last batch began at 0829. Full return at 0837 with grout at the top of the hole. Cascade began to pull the last 50’ of casing at 0937, and ACT topped off the hole at 0845.

QC test conducted at 0725 on “New A Grout Mix”: marsh funnel 38s. Potable Water Temperature 61.8 degrees F. Raw Water Temp 52.3 degrees F, Mud balance 1.43 g/cm^3.

2nd Test at 0826 “New A Grout Mix”: marsh funnel 35s. Potable Water Temperature 64.7 degrees F. Raw Water Temp 60.7 degrees F, Mud balance 1.39 g/cm^3.

ACT moved the grout cart to Test Section hole DXP2502 and initiated gravity grouting using the new A Grout Mix at 0936. The hole was topped off at 0939 and the head was held for 1 hour. No grout had to be added to maintain the head pressure. The hole was observed later in the day and there was at least 1 foot of water above the grout level. It is unclear if this was bleed or not. At 1039 ACT set up on hole DXP2482. This hole used the same grout batch as the previous hole. Grouting began at 1043 and the hole was topped off at 1050. The hole was complete at 1150 with only a fraction of additional grout required to maintain the head for 1 hour. This hole also was beginning to show the effects of bleed.

Cascade set up on hole DXP2262 at 0915. Embankment soils were as typically described in other holes with an organic zone identified at similar depths as noted in previous holes. At a hole depth of 105 fbgs 1 foot of sandstone was encountered under approximately 2 feet of weathered shale. USACE indicated this socket was insufficient so the driller went down again. The tools were advanced about 6 inches and a 5.6 foot soft zone was encountered. After the casing penetrated this soft zone the inner and outer casings bound together. This is attributed to rock fragments which wedged between the 2 pieces of casing.

Cascade attempted to free the casing from 1200 to 1430. Cascade tried to raise the casing up to free it but both pieces of casing moved together. 1 stick was removed, bottom of casing at 85 fbgs. Cascade asked the USACE what it preferred to move forward and USACE indicated that Cascade should seek direction from ACT. It was discussed that there is likely a plug of bedrock blocking the bottom of the inner casing.

Doug from ACT was briefed on the situation and it was explained that I have no contractual authority to change the contract or direct work and it would likely be Monday morning before the issue could be discussed and resolved with the proper individuals. Doug recommended that the entire drill string be removed and reinserted. This would result in an uncased hole which could have negative consequences.

We also discussed ways to abandon the hole, but even if the hole was abandoned with the casing in place there would still be an unsupported section of the hole approximately 10 fbgs which likely could not be grouted with the casing in place. Here are the options that I have identified: 1. Abandon the hole with steel in place. 2. Pull steel and then grout the hole up (in my opinion not desirable). 3. Pull the steel and then re-insert the casing back into the hole (definitely not desirable). 4. Use 8 in casing to go down around the drill string to support the hole from caving. If this could be advanced deep enough it is possible to free the inner casing and then safely re-install the casing to save the hole from abandonment.

Will Ailstock was contacted and it was decided to pull off the hole and relocate to DXP2382 so the issue could be discussed more in the AM. I left site at 1545.

11/9/15 - 11/15/15 Completed Work

Monday, 11/9/15:

Cascade on DXP2382. Installed sleeveport with a grout bag. ACT mixed the “bag grout” in the north batch plant (rather than the small portable mixer as they had done last week). The bag was left at pressure for 30 minutes. ACT mixed the revised A-mix, and pumped that into the annular space. After grout returned to the surface, Cascade removed the 7 inch steel, and then ACT later topped off the hole from the top.

They added a total of 100.8 gal including the top off.

Cascade then moved back onto DXP2262. Cascade had to do maintenance work on their drill head. At approximately 1230 they began to insert the 8 inch over-ride casing. Then they began to remove the drill rods. Cascade discovered the drill rods had broken at approximately 50 feet down and wedged against the 7 inch casing (causing them to be unable to remove the drill steel on Sunday). The crew was able to remove all the drill rods, the drill casing was empty. Cascade took a break at 1500 to eat lunch. *Note:

The 8 inch over-ride casing is at 90 feet. The 7 inch casing is at 100 feet. The hole was open to 100 feet.

After lunch Cascade reinserted the drill rods and began drilling. The driller had tried to get another 2 feet drilled. He had stopped his downward motion and tried to bring the rods back up, but he was unable to move upward. The driller said he was at about 103 feet. He thinks he is in a sandstone that is causing his rods to become stuck. He also thinks he needs to be drilling with some water (he would have his casing socked into rock a few inches before using the water). Cascade and ACT will discuss the best plan of action tomorrow. Cascade stopped trying to remove the drill rods at 1645, and the driller hopes that setting overnight might help him remove the rods in the morning.

ACT Gravity grouting at the top of rock interface using A-mix: DXP2462 took 21 gal; DXP2442 took 24 gal;

DXP2422 took 11 gal (a packer was inflated to prevent grout from coming up the PVC); DXP2402 took 6 gal (a packer was inflated to prevent grout from coming up the PVC).

Tuesday, 11/10/15:

Cascade is on hole DXP2262. Last night, the drill string was left in the hole (drill rods were stuck), with hopes the drill string would come loose after no activity through the night. First thing this morning, the driller was able to remove the drill rods and retrieve the sample. The rock was sandstone. The sandstone was destroyed by the vibrations of trying to remove the stuck drill rod and the vibrations of drilling. The bottom of the hole is at 107 feet.

An initial meeting was held at the drill rig. During the meeting we discussed top of rock determination and the amount of rock drilling, and discussed adding water as needed to help with drilling. ACT will send an RFI to confirm the discussions in writing.

After the meeting, Cascade installed the sleeveport PVC. ACT mixed the “Bag grout”. At 1020, the bag was inflated and the 35 psi pressure was held for 20 minutes. The bag took 6.9 gal of grout. ACT washed out all the lines. ACT then made the revised A-mix, the QC tested the grout at 36 seconds for viscosity,

1.38 g/cm^3 for the density, and 72 degrees for temperature. At 1132 they began grouting the annular space bottom stage. At 1209, they held pressure at 46 psi for 10 minutes. Grouting of the bottom stage took 66.3 gal. The first stage will be allowed to set for a minimum of 12 hours.

Cascade moved to DXP2242. They began drilling at approximately 1300. They drilled through the embankment without problems. The driller has been allowed to use a small amount of water to aid in preventing the soil from sticking to the casing and drill core barrel. They drilled from 105 to 115 feet between 1502 and 1505. The QC geologist and driller both thought they may have hit rock at the bottom of the run, but no rock was returned. When the driller began the next run, he immediately encountered rock – this run was drilled from 1520 to 1525. The bottom of the hole is at 118 feet. While examining the sample, we discovered the “rock” was actually concrete. The concrete appears “ratty”, and more non-structural.

The QC (Conrad), called Darryl on the radio to come out to rig to discuss. While we talked with Daryl, I suggested we may want a deviation survey on this hole just to make sure we aren’t going into the conduit. Darryl said the best way for them to run a deviation survey is to install the PVC pipe and grout it

in. The shift ended today after Cascade put the sleeveport pipe in the hole – ACT will grout first thing in the morning.

Wednesday, 11/11/15:

Cascade was set up on DP2242. ACT mixed up the “Bag mix” and at 0730 they began to grout the bag.

They held at 35 psi for 20 minutes and pumped a total of 7 gal (complete at 0800). ACT cleaned out their lines and batched A-mix. At 0836 they began grouting the bottom stage of the annular space. At 0901, the held pressure at 50psi. Grouting of the bottom stage was complete at 0910 after pumping 82 gal.

Cascade removed 60 feet of 7 inch casing.

Cascade then moved onto DP2262. After they got set-up, ACT had the double packer put into the hole to grout the upper stage (second stage) of the annular space. ACT had trouble locating the sleeveport with the double packer, and they had to deflate the packer and move up or down several times. At 1015, grouting began. At 1045 grout was at the surface. Cascade moved off the hole, and ACT topped off the hole with grout. They pumped 114.2 gal of A-mix, and grouting was complete at 1129.

11/16/15 - 11/22/15 Completed Work

Monday, 11/16/15:

Cascade set up on DP2362. They started drilling and pulled their first sample at 0742. They drilled through the embankment without issues. At 0832 they began their run from 65 feet. Top of rock was hit at 69 feet. The bottom of the hole is at 71 feet. They finished the run at approximately 0840, but they had to “work” the drill to pull the core barrel out (they were able to pull out at 0846). The rock was a competent sandstone. They ensured the bottom of the hole was clear, and then installed the sleeveport PVC. ACT mixed the bag grout mix, and began grouting the bag at 0942. The bag took 11 gal. ACT cleaned out the lines and batched A-mix. Annular space grouting was complete at 1112, it took a total 51 gal. Cascade then removed all the casing and moved off the hole.

Cascade set up on DP2242 to grout the upper stage of annular space. They began at 1211, but the packer was not set on a port, so they had to deflate and move the packer several times until they found the port. At 1225 they began annular space grouting. Grout was at the surface at 1247. Cascade removed the remainder of their casing and moved off the hole. ACT then topped off the hole. The second stage annular space took 46 gal of A-mix.

Cascade then set up on DP2182. ACT had discovered all previous holes drilled by Cascade had been drilled at a 10 degree angle. This hole was set up at 20 degrees. Began drilling at 1438. They stopped for the shift at 1608 due to concerns over holes near the conduit. Ended shift at 65 feet drilled (casing is also at 65 feet).

ACT then set up the televiewer cart to perform a deviation survey of DP2242. There are concerns about this hole (at 10 degree angle) and the conduit. This hole also had concrete from 115 feet to the bottom of the hole at 118 feet.

ACT set up this morning over DP2502 with the Core drill. They set up to drill the initial 15 feet of rock drilling. The first run was drilled from 17.25 to 27.25 feet (however the spec says the runs must be no greater than 5 feet). There was grout in the upper section of the hole. I spoke to Conrad about 5 foot runs. The final run was then drilled from 27.25 to 34.5 feet (end drilling at 0940). Adam also spoke to Mike Xu about 5 foot runs.

Tuesday, 11/17/15:

Cascade is still on hold from drilling DP2182. The hole is at 65 feet, with the 7 inch outer casing at 65 feet.

ACT set up the Cubex over DP2482. They rock drilled from 21 feet to 36 feet (with grout at approximately 19 feet). Completed hole at 0745. The geologist taped the hole at 36.2 feet.

Another deviation survey was performed on DP2242.

Wednesday, 11/18/15:

ACT was given permission to complete drilling DP2182. ACT will not be given permission to drill any other holes until a new QC plan is in place, and they have given us a plan to drill 20 degree holes to replace the holes previously drilled at 10 degrees.

Cascade started drilling on DP2182 from 65 feet. At 0920 the driller hit something hard at 131 feet.

When the sample was returned, there was a very stiff clay with rock fragments. At approximately 134 feet there was a chunk of rock and powder. The driller was told he needed to drill another foot at least.

He drilled to 138 feet. This sample returned with all clay and one little section of rock fragments at the top of the run. Drilling continued. On the next run, the top of rock was at 139 feet, and the bottom of the hole is at 141 feet (sample returned at 1030). Cascade then installed sleeveport PVC. The bag was grouted (8 gal Bag-mix) and then the bottom stage of annular space (108 gal A-mix). Cascade removed 7 inch casing, then moved off this hole and onto DP2362.

After setting up on DP2362, ACT grouted the upper stage (top stage) of the annular space. ACT again had to reset the packer several times before they actually set on a sleeveport. They could not find an actual port. They think they may have annular grouted over the port they were trying to find. The QC in IntelliSystem decided to just grout from the surface to get the annular space fully grouted. They started grouting from the top of the hole at 1418 (34 gal).

Thursday. 11/19/15:

ACT grouted the top stage of DP2182 this morning. They had grout at the surface at 0813. Cascade removed the rods and then ACT topped off the hole, complete at 0857. The top stage took a total of 95 gal of grout. Cascade is on hold until ACT is allowed to continue drilling.

The grout cart then moved down to DP2502. More A-mix was batched, lines were changed out (to make a smaller loop), and the mechanic had to come out to fix the cable in the grout cart. ACT began backfilling all the 10 degree angle holes at 1010. QA test performed on the fresh batch (Viscosity: 40 seconds, Density: 1.42 g/cm^3).

DP2502 – 17 gal DP2482 – 13.5 gal DP2462 – 19 gal DP2442 – 24 gal DP2422 – 27 gal DP2402 – 29 gal DP2382 – 35 gal DP2362 – 26 gal DP2302 – 50 gal DP2262 – 64 gal

At 1351, they finished backfilling all but DP2242. They cleaned out all their grout lines and then batched C-mix. They began grouting DP2242. Ended grouting at 1449 with 68 gal.

11/23/15 - 11/29/15 Completed Work

Monday, 11/23/15:

Held revised prep meeting for soil drilling and casing installation at 1100. Began soil drilling in hole DXP2502 at 1357 and completed at 1425. TOR was at 18 feet and hole was completed at 20 feet bgs.

Will begin grouting of barrier bag tomorrow in the am.

Tuesday, 11/24/15:

The contract was having problems with the grout plant in the morning. The barrier bag was installed in Hole DXP2502 and the packer was inserted into the hole at 0826. The barrier bag was inflated starting at 0839 and held at 35 psi for approximately 20 minutes. The barrier bag grouting was completed at 0859. ACT calibrated the instrumented packer between 0930 and 1000. The annulus grouting began at 1013. ACT pulled their drill steel and the hole was topped off at 1025 with a total of 20 gal of grout.

Cascade moved off the hole and the grout cart moved onto the hole. The instrumented packer was lowered into the hole and the soil-rock interface grouting begat at 1114. The interface grouting was complete at 1146 with 2 gal of grout.

Cascade set up on hole DXP2402 and began drilling at 1110. Top of rock was encountered at a depth of 45 feet at approximately 1155. The hole was finished at a depth of 47.5 feet. The barrier bag was then inserted into the hole and grouting began at 1239. The barrier bag grouting was completed at 1310 with a total of 4.49 gal. Annulus grout of DXP2402 began at 1347 and ended at 1413. The casing was then pulled and the hole was topped off at 1450. The sonic rigged was moved from the hole and the grout cart was pulled over the hole to complete the interface grouting. The contractor had trouble getting the packer down into the hole. They measured the hole at a depth of 29 feet and discovered that some grout had apparently leaked around the packer and caused some of the casing to be filled. The packer was unable to get through the grout so ACT filled the hole to the ground surface to gravity grout the interface. The hole was topped off to the top of the casing at 1532. At approximately 1602 the hole was washed keeping the bottom of the hose 1 ft. above the bottom of the hole to prevent washing out the soil rock interface.

Wednesday, 11/25/15:

No below ground work. ACT is setting up the core rig on DXP2502 and the Cubex on DXP2402 to begin coring and drilling in rock next Monday. The rest of the day was spent winterizing the equipment and grout plant for the Holiday.

Thursday. 11/26/15: No Work

Friday, 11/27/15: No Work

Saturday, 11/28/15: No Work

Sunday, 11/29/15 No Work

11/30/15 – 12/6/15 Completed Work

Monday, 11/30/15:

Began rock coring first 15 ft. on hole DXP2502 at 0825 and completed at 0915. (Cored from 20 feet BGS to 35 feet BGS.) A 2 inch clay filled joint was encountered at a depth of approximately 27 feet BGS. The core rig was moved off the hole and the hole was optically and acoustically televiewed. ACT tried to set up the CUBEX on hole DXP2402 and was having mechanical issues. The rig was repaired and drilling began at 1125 and was completed at 1150. The bottom of the hole is at approximately 63 feet BGS. The hole was then optically and acoustically televiewed. ACT will begin the gravity grouting of the holes tomorrow morning.

Tuesday, 12/1/15:

ACT had issues with the grout plant batching correctly in the morning. The grout plant was repaired and the B-mix was batched at 0810. The grout cart was set up on hole DXP2402 to gout the upper 15 ft. of rock. The packer was inflated at 0836 and the grouting was completed at 0905. The total grout take on the hole was 12 gal. ACT then set the grout cart up on hole DXP2502. The packer was inflated at 0935 and grouting the upper 15 feet was completed at 1007. The total grout take on the hole was 8 gal.

Tomorrow ACT will resume rock coring and drilling in the two test section holes.

Wednesday, 12/2/15:

ACT had a delay for all rock drilling. Initially waiting for 24 hour grout wait times but then they realized the Cubex drill had a metric spacer for the rods instead of one calibrated in feet. This would cause depth discrepancies and also make logging the holes in gINT difficult. The Boart diamond rotary drill starting running at 1100 on hole DXP2502. The first sample recovery from the 15 feet of gravity grouting was not competent grout (See Photo 1 below). The second sample contained competent grout that had set along with the first run of rock core (See Photo 1 below). PDT is checking with CD to see what type of water activities occurred in the hole after the grout was placed to avoid this situation occurring again.

Cubex drill started drilling at around 1230 on hole DXP2402. Not sure of depth, but at one point the driller stopped drilling and checked with Doug due to hitting a 1 ft. clay seam which was followed by 4 to 5 ft. of “crappy” rock. They continued to drill and ended up in the Sandstone with no further issues.

There were some rod drops but no water loss during that weak rock zone.

STA 24+02

14:30 - Reviewed Rizzio Field Logs at Sta 24+02 and discussed the drill hole.

15:25 - Completed Down Hole Water Hammer at Hole 24+02. No features of concern were noted in the Beech Creek LS or the Reelsville LS. The hole encountered the Sample SS at termination. The hole was flushed with the drill string in the hole. I did not record the total depth of the hole.

16:20 to 16:30 Circulated water to get clear water return.

Sta 25+02 Observed rock coring from approximate depth 56.6' to 145.5'. 16:45 last run of rock core was obtained at BOH ~145.5'. No water loss or rod drops were recorded on this interval. The soft zone noted in hole 24+02 between 96'-98' correlated to a shale zone at Sta 25+02. Other shale seams were noted above the beech creek limestone which also correlated to soft areas noted at hole 24+02 based on discussions with Rizzio. The core barrels are metric and have a length of 3 m, or 9.84 feet. Core boxes and logs are labeled and recorded to reflect these dimensions. The Logger from Rizzio measured, photographed, and determined RQD of the samples in place. Each piece of core was then moved individually to the core box and logged. The logs appeared to include adequate detail of the rock characteristics. I did not correct the fact that the logs were not completed before rock being placed in the core boxes because production in the SS above the beech creek went very quickly between 65'-98'. Future rock core logging should occur prior to boxing the rock core samples. It was also noted that the rock core was moved and stored in the downstream storage area, an unheated tent with no open sides to protect from the wind.

The contract requires that the rock cores be preserved in a controlled environment and not be allowed to freeze.

Notes by Steven Shifflett

Thursday. 12/3/15:

The contractor completed portions of the water pressure testing on DXP2402 from 1155-1632

Friday, 12/4/15:

ACT complete the water pressure testing DXP2402 from 0811-1046. Grouting of the assigned stages on 2402 began at 1240 and was finished for the day at 1610.

ACT completed water pressure testing on Hole DXP2502 from 1132-1623.

Saturday, 12/5/15:

ACT finished the final grouting of DXP2402 from 0805-0916. This included the remaining pressure stage, 15 foot gravity stage, and standpipe filling. Hole DXP2402 is now complete

ACT finished the water pressure testing on DXP2502 from 0741-0756. The grout stages were sent to ACT the previous evening. The grouting of 2502 began at 1020. The packer became stuck in hold DXP2502 at approximately 1215 and was finally removed at 1348. Grout Cart 1 was removed from the hole and Grout Cart 2 was set up. Grouting was resumed at a depth of 85.5 feet at 1405. The pressure grouting was finished at 1513 and began gravity grouting of the upper 15 ft. of rock. Grouting was completed at approximately 1645.

The first stage of the annulus took 38 gal. Grouting complete at 1703. Cascade then removed 20 feet of casing. Ended shift. ACT cleaned out lines.

ACT had an extra QC at the rig with a check list. The QC verified the angle of the rig several times. The QC that logs the drill samples also has another QC onsite helping. ACT included a filter fabric on the bottom of the PVC pipe for DP2382.

Tuesday, 12/08/15:

Started shift with sonic drill already set up on DP2382. The bottom (first) stage of the annular space grout was completed Monday. ACT grouted the upper (second) stage first thing this morning. Completed grout at 0850 with 18 gal. Cascade removed the casing and moved off the hole. ACT then grouted the rock interface from 0857 to 0944 using 0.42 gal.

Sonic drill set up on DP2462. Began drilling at 0859. Encountered rock at 0916 at 22 feet. The bottom of the hole is at 25 feet, reached at 0922. The bag was grouted starting at 1014, using 14 gal. The annulus was grouted using 10 gal, completed at 1118. Cascade removed the casing and moved off the hole. ACT then grouted the interface from 1123 to 1233 using 0.18 gal.

Sonic drill set up on DP2422. Drilled from 1140 to 1230, with the top of rock at 35 feet and the bottom of the hole at 37 feet. ACT had a problem with their packer (grout leaking out between the double packers). After waiting on repairs, ACT brought over a different grout cart with a working packer. They began grouting the bag at 1452. The bag took 4.4 gal. The annulus took 26.4 gal, and grouting was complete at 1608. Cascade removed all casing and moved off the hole. ACT then grouted the interface from 1646 to 1719 using 1.2 gal.

Sonic drill then set up on DP2362. They got the rig all ready to start drilling first thing in the morning.

Wednesday, 12/09/15:

Truck mounted sonic rig was already set up on DXP2362 this morning. Began drilling at 0709. Concrete was encountered at 68 feet. Rock was encountered at 69 feet. The bottom of the hole is at 70 feet (end drilling at 0847. The sleeveport pipe was inserted, and the bag was grouted starting at 0909. Once the pressure reached target, pressure was held for 20 minutes. The bag took 4 gal of “bag mix” grout. The cart and lines were washed out, and A-mix was batched. Grouted the bottom (first) stage from 1004 to 1019. The stage took 50 gal. Cascade removed 30 feet of casing, and moved off the hole. The interface was grouted between 1053 and 1126, and it took 6.8 gal.

Truck mounted sonic rig set up on DXP2322. Began drilling at 1058. At 1223, the driller began the run from 75 feet. The driller said it felt like he was in rock, and at 1225, he had drilled two feet and was stuck. He “worked” the rods, and at 1245 he was unstuck. The sample did not contain any rock. He went back in the hole to drill another two feet. Top of rock is at 77 feet and bottom of the hole is at 79 feet.

The crew inserted the sleeveport pipe with a filter on the bottom. ACT mixed the “bag mix” in the portable mixer (which they had moved near the rig). Began grouting the bag at 1403 – it took 5.6 gal.

The first stage (bottom stage) of the annular space took 52.4 gal of A-mix. Cascade then removed 40 feet of casing. The rig remained over the hole so the crew can come in first thing tomorrow to grout the second stage. ACT grouted the interface. Began interface grouting at 1537 and ended at 1655. This interface took 44 gal of A-mix. The flow was approximately 1.5 gpm when the grouting started and had gone down to approximately 0.1 gpm at the end of grouting.

Another Cascade sonic rig arrived on site this morning (a truck mounted rig). The crew set up on DXP2162. Drilling began at 1445. They ended shift at 1750 at 85 feet.

ACT moved a Cubex and a grout cart into the test section area at 1710.

Spoke with the lead engineer/geologist about drill log format. The drill log depth scale and the descriptions column both need better formatting (the lead is already working on these issues). The log depth scale needs incremental depth markings, not just depth markings at rock changes, and the depth scale needs better ticks. The description column will have a horizontal line to depict lithologic changes as well.

Thursday. 12/10/15:

The track mounted sonic rig began shift set up on DXP2322 ready to grout the second stage of the annular space. Grouting began at 0742 and completed at 0850, using 121 gal of grout. The casing was all removed and Cascade moved off this hole.

The track mounted sonic rig set up on DXP2362 to grout the second stage of annular space. Grouting began at 0851 and completed at 0925 – taking 18.5 gal. Cascade then removed all the casing and moved off this hole.

The track mounted rig set up on DXP2282. They began drilling at 1015. They encountered the top of rock at 93 feet (at 1155). The bottom of the hole is at 97 feet (drilling complete at 1212). Cascade inserts the sleeveport pipe with fabric on the bottom. Since ACT is grouting on the other sonic drill hole, ACT is now using two grout carts. ACT brings grout cart 19-10-05 and begins grouting the bag at 1315. The bag took 15 gal of the “bag mix”. Completed the first stage of the annular space grouting at 1441, taking 59.5 gal of A-mix. Cascade removed 40 feet of casing and moved off the hole. ACT then set up to grout the interface (1543 to 1622), taking 1.7 gal.

The track mounted sonic rig set up on DXP2242. Began drilling at 1535. They drilled embankment to the end of shift at 1749 (at 105 feet).

The truck mounted sonic rig was set up on DP2162 this morning at 85 feet. At about 134 feet, they hit a 2-3 inch piece of rock. From 1107 to 1117, they drilled from 135 to 145 feet. Weathered shale and soil was at 137 feet, and then good competent rock was at 141 feet (Top of rock 141 feet). The bottom of the hole is at 145 feet. Cascade then inserted the sleeveport pipe, with fabric on the bottom. The bag was grouted starting at 1230, taking 5.3 gal. The first stage of annular grout took 86.8 gal, completed at 1440. Cascade removed casing and moved off this hole. ACT then grouted the interface, taking 4.1 gal, from 1610 to 1645.

The truck mounted sonic rig moved onto DXP2102. They began drilling at 1624. They drilled through embankment until the end of shift at 1737, the hole is at 45 feet.

Friday, 12/11/15:

The track mounted sonic rig was set up on DXP2242 at the start of shift at 105 feet. They began drilling at 0814, and encountered rock at 109 feet. The bottom of the hole is at 113 feet. Ended drilling at 0819.

Cascade inserted the sleeveport pipe with fabric on the bottom. Grout cart 19-10-05 then pulled up next to the sonic rig, and the bag was grouted using 4.8 gal. The first stage of the annular space was grouted using 82.7 gal of A-mix (ended grouting at 0917). Cascade pulled out casing and moved off the hole. The grout cart set up over the hole to grout the interface. Began grouting at 1053. The target max pressure was set at 65 psi. The cart operator slowly increased his flow to about 6 gal per minute, but the pressure was only at 60 psi. At 1101, grouting stopped to allow for the time to get another batch of A-mix. At 1111 grouting continued and the target pressure was reached at approximately 3.5 gpm flow. Shortly after, grout had come up the inside of the PVC. Grouting stopped. The grout was flushed out of the inside of the PVC. At 1445, the grout cart set up over the hole and grouted the annular space from the top of the hole. At 1457 grout was at the surface – the second stage of annulus grout took 47.6 gal.

The track mounted sonic rig set up on DXP2522. They began drilling at 1120. The top of rock was 8 feet.

The bottom of the hole was 15 feet. The bag took 6.4 gal of “bag-mix”, the annulus took 12.8 gal (completed grouting at 1344). Cascade removed the casing and moved off the hole. The interface was grouted from 1355 to 1427 – taking 1.2 gal.

The track mounted sonic rig set up on DXP2342. They began drilling at 1438.The top of rock was encountered at 72 feet, and the bottom of the hole is at 75 feet (ended drilling at 1605). Cascade put the sleeveport pipe in the hole with fabric on the bottom. This hole will be grouted first thing in the morning.

The truck mounted sonic rig was set up on DXP2102 at 45 feet at the start of shift. Eight inch casing was used down to 105 feet, then Cascade put in the seven inch casing below 105 feet. The drill sample from 135 to 145 feet was a very plastic silty clay. At approximately 140 feet, the sample was a moist/wet sand. The top of rock was at 158 feet. The bottom of the hole was at 163 feet. The sleeveport pipe with fabric was inserted. The bag was grouted using 17.2 gal (ACT had trouble getting the double packer at the right depth for the correct port). The first stage of the annulus was grouted using 76.7 gal (ended at 1609).

Saturday, 12/12/15:

Track mounted rig started shift set up on DXP2342, the hole is at full depth at 75 feet with the sleeveport pipe inserted. Grout cart 19-10-05 moved over next to the rig. Began grouting the bag at 0726, using 5 gal of “bag mix”. The first stage of the annulus was grouted using 47 gal (complete at 0825). Cascade then removed casing and moved off this hole. The grout cart then set up over the hole to grout the interface (0912-0949). The top of rock took 4.8 gal.

Track mounted rig set up on DXP2302. Drilling began at 0928. Drilling continued through the embankment without problems. The run from 85 feet began at 1119 and completed at 1121. The top of rock is at 85 feet and the bottom of the hole is at 87 feet. The sleeveport pipe was inserted (with the fabric on the bottom). The bag was grouted starting at 1205 (taking 4.5 gal). The first stage of the annulus took 56 gal (complete at 1325). Cascade removed casing and moved off the hole. The grout cart (19-10-05) set up over the hole to grout the interface. Grouted 3 gal from 1415 to 1451.

Track mounted rig then set up on DXP2262. They began drilling at 1415. The run from 95 to 100 feet was between 1612 and 1614. The top of rock is at 100 feet. The run from 100 to 104 feet was from 1626 to 1627. Bottom of hole is 104 feet. End of shift.

The truck mounted rig was set up on DXP2102. Grout cart 19-10-07 grouted the second stage starting at 0736. It took 218 gal and completed at 0913. Cascade then removed the rest of the casing from the hole. Cascade waited for the grout cart to complete second stage grouting of DXP2162 before they moved off this hole and set up on DXP2162.

Grout cart 19-10-07 set up over DXP2162 to grout the second stage. ACT was unable to find the port to grout this stage. They grouted the second stage from the surface of the hole. Grouting began at 1050, and it took 151 gal. Cascade then set up over the hole and removed the casing. ACT topped off the annular space twice (1200 and then again at 1325). The first top off took 89 gal and the second top off took 56 gal.

Grout cart 19-10-07 then grouted the interface of DXP2102. Began at 1117 and completed at 1154. It took 1.4 gal. The grout cart then grouted the interface of DXP2162 from 1509 to 1645 – taking 4 gal.

The truck mounted sonic rig set up on DXP2042. Drilling began at 1207. At 1420 (on the run to 65 feet), the bolts on the head of the rig snapped. Cascade was able to fix the rig – and drilling resumed at 1600.

Reached 85 feet (embankment) at end of shift.

Sunday, 12/13/15:

The track mounted rig started shift set up on DXP2262 already at full depth and with the sleeveport in the hole. Grout cart 19-10-05 was at the rig and began grouting the bag at 0740. The bag took 4 gal.
